Passive Voice
We only use the passive when we are interested in the object or when we do not know who caused the action.
Example: Appointments are required in such cases.
We can only form a passive sentence from an active sentence when there is an object in the active sentence.

To form a sentence in Passive Voice we need:

Verb to be (am, is, are, was, were, be, been or being)
+
Past participle of the verb (required, watched, built, seen, written, made, sent, etc.)

Examples:

Active: Peter builds a house.

Simple Present
Passive: A house is built by Peter.

Active: Peter built a house.

Simple Past
Passive: A house was built by Peter.

Active: Peter has built a house.

Present Perfect
Passive: A house has been built by Peter.

Active: Peter will build a house.

will-future
Passive: A house will be built by Peter.

Active: Peter can build a house.

Modals
Passive: A house can be built by Peter.

Conjugation for the Passive Voice

We can form the passive in any tense. In fact, conjugation of verbs in the passive tense is rather easy, as the
main verb is always in past participle form and the auxiliary verb is always be. To form the required tense, we
conjugate the auxiliary verb.
For instance:
 present simple: It is made
 present continuous: It is being made
 present perfect: It has been made
 past simple: It was made
 past continuous: It was being made
 past perfect: It had been made
