You will complete a final paper (100 points), due the last day of class by midnight. 5
points will be deducted for each day or partial day the paper is late.

In this paper: Your goal is to write a narrative form evaluation & intervention plan.
You want to demonstrate you can take a case study and use practice models and
evidence based practice to develop and justify this plan in cogent and comprehensive
manner. You can identify multiple areas of concerns based on the practice models
you chose to use and then identify your priorities for intervention.
 Select a case that we have used in this class this semester.
 You may consult with each other; your paper must be your own work.
 Your paper will be 5-7 pages typed, double-spaced, 12 font, 1-inch margins,
and using complete sentences, unless chart is approved.
 Use in-text citations in APA format with a reference list for models, research
and OTPF (AOTA, 2014).

Write an Occupational Profile (Summary of client & reason why seeking
service, from the AOTA template) and a summary of Analysis of Occupational
Performance (interpretive narrative) for the client you chose. This part of

supporting your analysis that helped you develop your summary narrative
for the occupational profile and analysis.

Explain and detail your targeted occupational outcomes & priorities stated in
your narrative relating them to outcomes in OTPF (AOTA, 2014, S34-35).
4. 5 Pts. – Priorities (Narrative) 5 points
Identify two occupational priorities that you will address in intervention.
Explain how and why you chose these priorities among the various
barriers/weaknesses you identified above in your detailed domain analysis.

P: Intervention Plan:
5. 30- Pts. Practice Models (Narrative) 29 points
Incorporate & apply your intervention evidence by choosing:
a. 1 Organizing practice model (EHP, OA, MOHO, PEO, LR)
b. 2 complementary practice models (Biomech/rehab, motor
control/motor learning, DIM, social participation, Kawa, Life Balance)
to apply to your case study to generate ideas about occupational
therapy assessments and intervention.
Choose models to ensure you are developing a comprehensive & holistic
evaluation & interventions for your client.
o Explain the models and justify their relevance, using postulates of
change
o Write a rationale & explain how and why you chose these models to
address priorities and areas of intervention you chose in this case. Use
research articles (site in APA format) in your narrative to support
your rationale.
o Tie the models back to the outcomes you chose and the two priorities
of intervention that you identified.
6. 10 Pts. –Goals – (Narrative) 10 points
Using OTPF (AOTA; 2014) concepts and the terms of your models, choose
one of your stated priorities and write an occupation based Long Term Goal
(LTG) for this priority with 2 supporting Short Term Goals (STGs) in SMART
Format.
7. 10 Pts. – Intervention Approaches & Types – (Narrative) 9 points
Identify 2-3 intervention approaches (p. S33) and 2-3 intervention types (p.
S29-S31) according to the OTPF (AOTA, 2014), research evidence and
practice models that support your intervention. Using in-text citations in
APA format, briefly summarize the research evidence that supports your
intervention
8. 10 Pts. – Intervention Session 10 points
Describe an intervention session that is approximately 45 min. in length that
addresses your stated goals.
