Longitudinal Activities and Applications


Extension of Patient Profile
By October 8, 2018 at 5 p.m. (upload to Dropbox)

 Revive your patient profile from spring 2018 and include revisions based on recommendations you made from your SOAP
note at that time (unless you now conclude that your recommendations were not sound, in which case revise accordingly).
Please note that your patient profile must have exactly the information specified at the bottom of these instructions.
 In addition to your other chronic conditions, you’ve unfortunately developed Type 2 diabetes. You were diagnosed August
20, 2018 with a fasting glucose level of 172 mg/dL and symptoms including polydipsia and nocturia; your hemoglobin A1C
was 9.3% on August 20, 2018.

o Decide on basic lifestyle modifications you will implement (meal planning, physical activity), but don’t expect you will
adhere perfectly, because most patients do not. Perhaps you will not be motivated to initially modify the quantity of
chocolate you consume, for example. You know yourself, so can decide what your struggles will be.
o Start yourself on ONE medication for use in Type 2 diabetes, taking into consideration your co-morbid conditions, etc.
Use the objective data provided for your P-2 SOAP note as the most recent evidence of your organ function. Decide
the starting dosage, frequency and when you will administer the agent.
o Start yourself on a regimen of glucose monitoring --- decide how frequently and at what time you will monitor

 Unless otherwise directed, upload your revised patient profile to the course D2L Dropbox by October 8, 2018 at 5 p.m. and
bring it to laboratory sessions as directed. Each student must have exactly the following information on his/her profile:

o 5 disease states (4 from your P-2 year, plus Type 2 diabetes)


o 5 prescription medications (4 from your P-2 year, plus the new agent for Type 2 diabetes)
 If you are finding it difficult to maintain 5 prescription medications, consider tactics such as the use of
combination antihypertensive agents, which would still count as only one medication on your profile
o 1 or 2 non-prescription agents with dosage/frequency and purpose of use (can include any OTC products, vitamins
and/or herbal products, your choice)

You do not have to write a SOAP note based on this new information. Just ensure your patient profile is updated by the deadline and
contains exactly the information requested. You should have your patient profile accessible when scheduled for labs later this fall
semester and throughout the spring semester. You will also be entering your profile information into a survey during the fall term.
