AFFIDAVIT OF DISCREPANCY OF YEAR OF BIRTH

I, JULIETA ARIB RIEZA, of legal age, married and with address at Brgy. Abo-Abo,
Sofronio Espanola, Province of Palawan, Philippines, after having been duly sworn to in
accordance with law, hereby depose and say, state:

1. THAT the date of birth appearing my Certificate of Live Birth issued by office
of the Philippine Statistics Authority is “July 1, 1961”;

2. THAT the date of birth appearing in my Professional Identification Card with

number 0510226 was inadvertently entered as “July 1, 1960”;

3. THAT I am actually born on “July 1, 1961” at Aboabo, Brookes’ Point,

Palawan;

4. THAT I am executing this Affidavit to attest to the truth of the foregoing facts
and to use the same for whatever legal purpose it may serve.
