CONTRACT OF EMPLOYMENT

This Contract is made and entered into, this _____ day of January, 2012 at Angeles City,
Philippines, by and between:

______________, a corporation duly organized and existing under and by

virtue of the laws of the Republic of the Philippines, with principal office at
________________________________ duly represented by its ________________,
_____________________________ (hereinafter referred to as the “EMPLOYER”);

- and -

___________________, of legal age, Filipino citizen, with postal address at

_________________________________ (hereinafter referred to as the
“CONTRACTUAL EMPLOYEE”)

WITNESSETH: That -

WHEREAS, CONTRACTUAL EMPLOYEE has applied with _______________ to be

employed on a fixed term basis;

WHEREAS, _______________ has accepted the offer of CONTRACTUAL EMPLOYEE to

be employed on a fixed term basis.

NOW, THEREFORE, for and in consideration of the foregoing premises and the mutual
covenants herein set forth, the parties have entered into this Agreement and have agreed, as
they do hereby agree, as follows:

1. CONTRACTUAL EMPLOYEE is hereby appointed to work as a

CONTRACTUAL EMPLOYEE of EMPLOYER with the position of _______________.

2. This contract shall be in full force for a fixed term of ________ (___) year, from
__________________ until _____________, unless sooner terminated by either party for lawful or
just cause(s) as provided by law and/or the policies, rules and regulations of _______________,
which may now or hereinafter be enacted or promulgated, including, but not limited to earlier
completion of the work for which CONTACTUAL EMPLOYEE was hired, non-compliance with
work standards, rules and regulations, substance abuse (drugs or alcohol) or for any other
justifiable reason.

This contractual employment shall be limited only for the period above indicated and
shall automatically terminate on the date of expiration as above stated without need of any
further notice.

3. For and in consideration of this appointment, CONTRACTUAL EMPLOYEE

shall receive a corresponding monthly compensation which is based on a total of forty eight (48)
hour work week:

Basic Rate/Month

The monthly compensation and/or Cost of Living Allowance (COLA) shall be subject to
the adjustments and/or deductions as may be required by law, including but not limited to
Wage Orders, Pag-ibig, Philhealth, SSS, and Withholding Tax.

4. This appointment shall be subject to, and the CONTRACTUAL EMPLOYEE

hereby undertakes to abide by, the policies rules and regulations of _______________ which
may now or hereinafter be enacted or promulgated, and other pertinent laws.
