I, JO1 Rico R Calo 32 years old, married, residing at 144 Santan St., Brgy. Villa Kananga,
Butuan City, Agusan del Norte after having been sworn to in accordance with the law do
hereby depose and state:

That I am currently assigned in Bayugan City Jail, primarily designated as Custodial and
Escort personnel and a duty mate of JO1 Carla Tosca Erika V Guittap during the alleged
beating incident on January 8, 2016 of inmate Rosalyn L. Perolino, a detainee of the same
jail.

That I am stating the foregoing facts according to my actual knowledge without any force
or influence from any person:
