AFFIDAVIT OF OWNERSHIP

THAT I, ROCELINE B. JONSEWAR, of legal age, Filipino, married, and a

resident of Bongao, Valencia, Negros Oriental, after having been duly sworn to in
accordance with law, do hereby depose and manifest:

That I am an owner of a 29” POLYTRON MULTI SYSTEM Telivision with 181

channels; type 29PS2000 with Serial No. AB-00447/13 ;

That the purchase price of said Television was sixteen thousand Pesos (Php16,000)
Philippine Currency;

That this affidavit is executed freely and voluntarily and to attest the veracity and
truthfulness of the aforementioned statements and not to defraud any other third party.
