Re: Thomas Gonzalez

Mr. Gonzalez is a 52-year-old male who will be making an appointment to see you in the very near
future.

He has had emphysema for many years, and it has become progressively more severe. At this time, he
presents with complaints of increasing weakness, particularly with minimal exertion. This is undoubtedly
related to his severe emphysema.

I have not instituted any change in his therapy, since he plans to see you with the week and follow your
recommendations.
