Golden Shower, Pagadian City after having been duly sworn to in accordance with law hereby
depose and state that:

I was the respondent to the Special Proceedings Case No. 13-016 for Annulment of
Marriage at the Regional Trial Court 9th Judicial Region, Branch 29, Pagadian City;

The said case was already decided last January 21, 2014 which granted the annulment
of my marriage with Allan Carbonel on August 8, 2009 in Bayugan Agusan del Sur;

I hereby WAIVE my rights with regard the properties owned by my husband, to wit:

physically and emotionally suffered during our marriage because of my sexual orientation as a
