AFFIDAVIT-EXPLANATION RE DISCREPANCY
IN BIRTH DATE

I, SERAPIO BOTE VELASCO, of legal age, single, Filipino and with

residence at Brgy. Canlapwas, Catbalogan City, after having been duly sworn to in
accordance with law do hereby depose and state that:

1. I am a government employee assigned at the Office of the

Provincial Warden, Catbalogan City, as Prison Guard 1;

2. I was born on 16 November 1965 at Brgy. Buenavista, San

Jorge, Samar, to the late Sps. Rosalio Villascosa Velasco and
Cresencia Berreber Bote;

3. Unfortunately, due to my parents’ negligence, my birth was not

registered with the Local Civil Registry, San Jorge, Samar, although I
was baptized at the Parish of St. Michael the Archangel, Gandara,
Samar, on 23 January 1966; a copy of my Baptismal Certificate is
hereto attached and made a part hereof;

4. With said baptismal certificate and other supporting documents,

I effected the late registration of my live birth with the afore-mentioned
registry, and a Certificate of Live Birth was issued in my favor which
evidently bears, among others, that my date of birth is 16 November
1965; a certified true copy of which is hereto attached and made a
part hereof;

5. On 29 November 1992, I took the Professional Career Service

Examination in Calbayog City, conducted by the Civil Service
Commission (CSC) and with my passing mark, I worked in the
government office and until the present;

6. I consistently presented myself to have been born on 16

November 1965 in all my personal endeavors in society, particularly
in my baptismal certificate, GSIS, Pag-IBIG, LTO Driver’s License,
and in my employment records, and other personal identification
cards, among others;

7. Lately, I was gathering and collating my personal records and

credentials for my promotion, including my Certificate of Eligibility,
and to my surprise, however, it therein appeared, among others, in
said Certificate of Eligibility, that my date of birth is 16 November
1963 and not 16 November 1965, much to my surprise;

8. I tried to recollect such date, 16 November 1963, and it

occurred into my mind that probably it was because of my apparent
error to correctly indicate my birthday caused by my mixed feelings
then prevailing and that the carelessness or negligence on my part

 was compounded by my failure to carefully review my entries
therein which otherwise could have afforded me to rectify such error
accordingly and with dispatch;

9. I immediately went to the office of the Civil Service Commission

to verify, and therein found that my personal data records in the Seat
Plan during the examination date was really my true date of birth
which is 16 NOVEMBER 1965;

10. I believe the said error in indicating my date of birth as

appearing in my Certificate of Eligibility could have been due to
inadvertence of simple clerical error and I hereby relieve and release
the CSC from any and all obligations and liabilities, if there be any,
arising therefrom;

11. Consequently, to avoid confusion on my personal

circumstances and for purposes of convenience, I respectfully
request the CSC to accordingly amend my date of birth appearing on
its records, particularly on my Certificate of Eligibility, or from 16
November 1963 to 16 November 1966, in conformity with my
baptismal certificate and birth certificate which takes primary and
foremost precedence over all other documents pertaining to births;

12. Notwithstanding the discrepancies in my date of birth, the

same refers to my one and only person and no other;

13. The afore-mentioned discrepancy in date of birth is made in

utmost good faith and never intended to defraud the government, its
agencies and instrumentalities, particularly the CSC, and third
persons nor to evade obligations, but solely due to the valid reasons
therein set forth; and that I am fully aware that any false information
or statement herein and in any supporting documents shall expose
myself to administrative, criminal and civil liabilities;

14. I am executing this affidavit to attest the truth of all the

foregoing and to request the Civil Service Commission, Regional
Office No. VIII, Tacloban City, for the correction of my personal
information in my records therewith, particularly on my date of birth
afore-mentioned, and for promotion and other legal purposes.

 1. On 24 November 1985, I took the Professional Board Examination
for Teachers (PBET) in Borongan, Eastern Samar, conducted by the Civil Service
Commission; and that during that time, I was teaching in Our Lady of Fatima
Academy (OLFA), Gen. MacArthur, Eastern Samar;

2. After passing such board examination, I was employed as secondary

teacher at Wright Community School, now Wright National High School, Paranas,

continuously to date; that in good faith I adopted the place of birth, Paranas,
Samar, hoping that such will not affect my personal circumstance;

sheet, service record, and other personal identification cards;

City on 06 December 1971, and such fact, among others, was
indicated in my certificate of live birth issued and authenticated

3. On 25 October 1992, I took the Professional Board

Examination for Teachers in Catbalogan City conducted by the
Civil Service Commission; unfortunately, however, I erroneously

be 06 December 1971 which I adopted and presented and
consistently adopts and presents in all my personal
representations in public in all of my documents as evidenced
by a certification issued by the Office of the Local Civil Registrar,

of Samar, Catbalogan City; Official Transcript of Records,
Sacred Heart College, Catbalogan City; Official Transcript of
Records, Samar: GSIS ID Card; PHILHEALTH Member Data
Record, all hereto attached as Annexes “B” to “F, ” inclusive,
and made a part hereof;

4. I passed the said examination, and accordingly I was issued a

Certification of Eligibility on 30 March 2004 with my name “MA.
BELLA CECILIA B. GUY” entered in the Official Register of
Eligibles” (such name was corrected to MARIA BELLA CECILIA
B. GUY pursuant to my earlier petition with the Civil Service
Commission {CSC}); and with my earlier entries, it was
indicated that my date of birth is “06 December 1972, and not
“06 December 1971” which is my true and correct birth date, to
stress;

5. The erroneous information as far as my date of birth, which was

carried over in my certification of eligibility, could be attributed
to my mixed feelings then prevailing- pure excitement, tension,
nervousness and anticipation of the examination, as I prepared
my application; and that carelessness or negligence on my part
was indeed coupled by my utter failure to review my entries
contained therein which otherwise could have afforded me to
correct accordingly;

6. The said error in indicating my date of birth is solely attributable

to me or that the fault is mine alone, and I hereby relieve and
release the CSC from any and all obligations and liabilities, if
there be any, arising therefrom;

7. Consequently, to avoid confusion on my personal circumstances

and for convenience, I shall henceforth abandon my date of
birth from 06 December 1972 to 06 December 1971 and I
respectfully request the CSC to accordingly amend my date of
birth appearing on said certification, in conformity with the birth
certificate issued by the National Statistics Office which takes
precedence over all other documents, including that certificate
issued by the civil registry;

8. Notwithstanding the discrepancies in my date of birth, the same

refers to my one and only person and no other;

and such fact, especially my given name CRESENCIA, was
indicated in my certificate of live birth issued and authenticated
by the National Statistics Office (NSO) photocopy hereto
attached as Annex “A” and made a part hereof;

13. On 12 September 1954, I was baptized at Sts. Peter and Paul

Parish, Paranas, Samar, and thus was issued a certificate of
baptism which likewise indicates that my given name is
CRESENCIA, photocopy attached as Annex “B” and made a
part hereof;

particularly during my primary grades, my mother informed me
that my given name is spelled as CRESCENCIA and believing
her therefore, I had consistently adopted such given name in
all my academic undertakings as evidenced by my Bureau of
Internal Revenue Tax Identification No. 153-503-648, Home
Development Mutual Fund, Official Transcript of Records
(Doctoral Program), Samar State University, Catbalogan City,
GSIS Certificate of Membership CM. No. 3883765 dated 27 July
1999, as photocopies of which are hereto attached as Annexes
“C” to “F” and made a part hereof;

15. Moreover, on 20 April 1978, I took and passed the

Professional Board Examination for Teachers (PBET) in
Catbalogan City and was issued a Report of Rating by the
National Board for Teachers, Manila, on 15 February 1979, and
therein it appears, among others, that my name CRESENCIA
was misspelled as CRESCENCIA, photocopy of which is hereto
attached as Annex “G” and made a part hereof;

16. Be that as it may, there were instances that I presented my

given name as CRESENCIA in my representations and
documents as well, particularly in my marriage certificate, upon
confirmation that my given is indeed spelled as CRESENCIA as
gleaned in my birth certificate which I secured only on 02 May
2013 and not CRESCENCIA; and henceforth, and even prior
thereto, to avoid confusion on my personal circumstances, I
corrected my given name in my documents and identifications,
including but not limited to my Professional Regulation
Commission Identification Card as Professional Teacher, Social
Security System Identification Card, and SSU Employee ID,
photocopies attached as Annexes “H” to “J” and made a part
hereof;

17. Plainly put, at times, I interchangeably used my given names

CRESCENCIA, which is erroneous, and CRESENCIA, which
name is indicated in my birth certificate which I adopted
permanently; and notwithstanding the discrepancies in my given
name, the same refers to my one and only person and no other;

18. The afore-mentioned discrepancy in my given name is made in

utmost good faith and never intended to defraud the
government, its agencies and instrumentalities, particularly the
Civil Service Commission, Regional Office No. VIII, Government
Center, Candahug, Palo, Leyte, and third persons nor to evade
obligations, but solely due to the valid reasons herein set forth;
