Dear Sir:

Greetings!

I am writing this letter with the conformity of my client, ANNABELLE M.

LIBRES, hoping that she will get some valuable information from your good office
regarding her husband METODIO LIBRES who died while working on board
last October 2017. She was informed by a friend who experienced the same
desolation that they were able to receive from your end some form of financial
assistance.

Hence, we would like to request your good office if METODIO LIBRES, as

represented by his wife, ANNABELLE M. LIBRES can claim any form of
financial benefits/assistance from your organization. Be it known that upon the
loss of her husband, Mrs. Libres had been through financial drought and endless
law battles to fight for the benefits she is entitled, even until now, she had been
treated unfairly in terms of monetary assistance.

On the other hand, it is also not certain on our part that the late METODIO
LIBRES is a qualified member of AMOSUP or qualified to receive any form of
benefits since Mrs. Libres cannot find any document issued by AMOSUP from the
files of her late husband. Be that as it may, we will be glad if your office can inform
us about this matter.

Herewith attached are documents related to Mr. Metodio Libres’ work and fact of
death.

Any form of help will be much appreciated and in many folds you will also be
