Objectives
Periprosthetic fractures in hip arthroplasty are challenging because of the bone stock and bone
quality. The best solution for unstable fractures (Vancouver B3) is uncemented extensively porous-
coated femoral stem.
We present a retrospective analysis of 4 cases operated in our clinic during the last 2 years. The
mean age in our group was 67 years old (54-73). In three cases, the femoral implant was an uncemented
stem and in one case the patient had a cemented stem. The causing mechanism was a low-energy
trauma (same level fall) in two cases, in one case was a high-energy trauma (pedestrian car accident)
and in the last case no trauma was reported (periprosthetic fracture 6 weeks after uncemented total hip
artrhoplasty).
Results
All four patients had good short and midterm results. One patient suffered an abundant lavage
and secondary suture 7 days postoperative.
Conclusions
In our opinion uncemented revision with cables for type B3 fractures is the elective method of
treatment.
