prosecutor __________________ at the Provincial Prosecutor’s Office, Upper
Carmen, Cagayan de Oro City. However, for ______________________, I
resorted to a registered mail considering that his office was closed;

4. I personally served a copy of the Petition for Review thru registered mail at the
Office of the Secretary of Justice, Department of Justice, Padre Faura St.,
Ermita, Manila;

5. This affidavit is presented to attest to the truth of the foregoing facts.
