AUTHORIZATION LETTER

To whom it may concern,

This is to inform you that I, Mary Rose G. De Guzman, am authorizing my sister

Mary Grace G. De Guzman to claim my Postpaid Plan (Plan 1799 with iPhone 6 Plus) which
we availed on October 8, 2018.

I will not be able to claim the said services personally due to conflict in distance of travel
and work schedule. Attested in this letter are my valid IDs and other supporting documents of
my claim.
