Dear Parents/Guardians:

I wanted to take this opportunity to spread the word about the TeleHealth services we are
offering to all students & staff with Clarity Health Services. The benefits of this service are to
allow our students to stay in school as much as possible and to make it easier for parents as
they will not have to leave work for an appointment with a physician. The Nurse Practitioner I
will be working with would be able to see vital signs, look at throats, assess a rash, and check
the ears with special equipment I have at school to allow her to diagnose properly. I will also be
able to do a throat swab to test for strep and a swab to test for flu if the nurse practitioner orders
one to be done. She can call medication to your pharmacy of choice if they are needed. There is
a possibility that a student will still be sent home, especially if they are running a temperature or
are found to be contagious. Situations like severe abdominal pain (appendicitis), possible
fracture, anything that may need x-rayed or blood work, neuro complaints such as: (seizure,
muscle weakness, difficulty reading & writing) would be not be done at school.

This is a new integrative healthcare clinic inside Paris R-II schools. It is open for all students,
faculty & staff. The clinic services are free to any student who qualifies for free or reduced
lunches. Most insurance providers are also accepted. Sliding fee scale is available for those
who qualify.
Parent written permission is required before service is provided. Consent forms are available on
school website or school offices/nurse office.

Once Clarity has a student’s paperwork, it will be put into their system. When a student
presents with temperature, complaints of sore throat, nausea, headache, persistent
cough/congestion/wheezing, etc., I first will get verbal permission from parent/guardian to
proceed with calling the nurse practitioner. Once permission is given, the patient will be put on
the schedule. I will give all medical history to the nurse to start a chart in their system eg:(meds,
allergies, past medical history, etc.) When all of that is complete, I will be transferred to the
nurse practitioner to see the student through our telehealth computer system.
