Page $ of 81 i) the placement of @ person into, (A) a long-term care home within the meaning of the Long-Term Care Homes Act, 2007, (B) a supportive housing program funded by the Ministry of Health an Long-Term Care or a local health integration network under the Home Care and Community Services Act, 1994, (C) chronic care or rehabilitation bed in a hospital within the meaning of the Public Hospitals Act, or (D) en adult day program that is provided under the Home Care and Community Services Act, 1994, or (iii) any other services that are prescribed, and (@) any other organization that is provided for in ‘the regulations and that receives public fonding; (“organisme du secteur de Ia santé”) “health service provider” has the meaning set out in subsection (2); (“fournisseur de services de santé”) “integrate” includes, (a) to co-ordinate services and interactions between different persons and entities, (b) to partner with another person or entity in providing services or in operating, (c) to transfer, merge or amalgamate services, ‘operations, persons or entities,
