1 Explain the purpose and procedure to the patient.


2 Perform hand hygiene.
3 Prepare linens needed, assemble them and arrange on a bedside chair in the order
in which items will be used.
a. towel
b. pillow case

g. bottom sheet
4 Lock the patient’s bed and adjust to a comfortable working height. Lower side rails,
if present.
5 Don gloves. Check bed linens for any items belonging to the patient, like eye
glasses, dentures, or other belongings that may have fallen among the linens.
6 Loosen the linens/ beddings systematically, starting at the head part of the bed, on
the side and moving all around the bed ending up to the head part of the bed near
you.
7 Remove the pillow case, place pillow on the bedside chair.
8 Fold any reusable linen like the bedspread, and the blanket on the bed, into fourths.
Place them over the back of the chair.
9 Remove sheets separately by snugly rolling them with soiled part inside, hold all
soiled linens away from your body/ uniform, and place them directly into the linen
hamper.
10 Remove the rubber sheet and discard if soiled, but if the rubber sheet is to be
reused, roll or fold rubber sheet and place on chair.
11 Turn the mattress to the opposite side if necessary, and replace the mattress pad (if
present) as needed.
12 Remove gloves and Wash your hands thoroughly after handling soiled linens.
13 Place the bottom sheet with its center fold in the center of the bed, the hem of the
sheet is even with the edge of the mattress at the foot part.
Unfold the sheet across the mattress then unfold from the foot part toward the head
of the bed.
14 Tuck the bottom sheet securely under the head of the mattress on one side of the
bed, miter the corner and then tuck the sheet under the mattress. Miter the corner at
the head of the bed.
15 Place the rubber sheet evenly on the center of the bed or approximately at the
buttocks area (patient’s midsection) and unfold crosswise.
16 Put the draw sheet over the rubber sheet, unfold crosswise and tuck the surplus on
both sides together with the rubber sheet..
17 Move to the other side, and secure the bottom linens. Pull the remainder of the
sheet firmly so that there are no wrinkles.
18 Place the top sheet with the wrong side up wider hem at the head part in line with
the upper edge of the mattress and with the centerfold along the middle of the bed.
Tuck the foot part and miter the corner. Leave the sides free.
19 Place the blanket/ spread over the top sheet at about 8 – 10 inches from the top of
the mattress, with its center fold in the middle of the bed. Unfold crosswise and form

a cuff over the blanket/ spread and then spread the blanket towards the foot part.
20 Make a toe pleat, or vertical tuck, 2-4 inches in the top linens to allow room for the
patient's feet and to prevent pressure that can cause discomfort, skin breakdown,
and foot drop.
21 Tuck in the excess sheet and blanket and make corners.
22 Go to the other side of the bed, and secure the top beddings in the same manner.
starting from the foot part of the bed and tuck all linens under the bed
23 Put clean pillowcase. Grasp the closed end of the pillow case at the center with one
hand. w/ the free hand, pull the pillow case over the whole pillow
24 FOR OPEN BED:
- Open one side of the bed by folding (FAN-FOLD) the top covers back where the
patient will be transferred..
FOR CLOSD BED
- Tuck all the linens at the corners and sides of the bed
25 Hang the bath towel/ hand towel on the rack/ head part, and place the call button
within the patient’s easy reach.
26 Adjust bed to the low position if the patient is returning to bed after being up.
Leave the bed in the high position if the client is returning by stretcher, Straighten
bed, bedside table and replace chair
27 Dispose soiled linen according to agency policy, remove gloves if used and wash
your hands thoroughly.
28 Applied Proper body mechanics accurately during the procedure.
