The tools and equipment used: SCISSER LIFT /BOOMLIFT,SCAFFOLDS ,

The training and skill required for the job: Tool box meeting, Training for hot work regarding safe work procedure and work briefing with all workers.

Personal Protective Equipment Required: Safety Shoes, Hard hat, Hi-vis Jacket, Safety glasses, Shield, leather hand gloves, Nose mask, Safety Harness.

ACTIVITY HAZARD
CONTROL MEASURES

a) Permit to work to be implemented And displayed at work area.


b Daily toolbox meeting and risk assessment briefing to be conducted
Requirements Prior to start work.
c)checklist for machinery, equipment and tools to be updated
And in place before work start.
d) Housekeeping to be done at the end of the day work.
e) Pre-inspection of machinery and equipment before mobilization of
Machinery to site.
f) All workers to be provided with proper Personal Protective
Equipment
g)Lifting rigger LOA, Lifting supervisor LOA & Signal man LOA need
to submit before starting then lifting rigging work

2.Electrical hazards 2.1.Machine is inspected and colour coded EHS personnel,

electrical tools.
2.4. To check on physical condition of cable to ensure it is free
from defect before use.
2.5. Ensure all electrical tools equipped with a dead man switch.
2.6. Always use ELCB of 30mA when connect to main power
supply.
2.7. Ensure that all plugs and sockets used are of the approved
industrial type.

3.Mobilization 1.Collision with People and Property 1.1.Scissor lift /Boom lift shall be inspected by M+W EHS
s of scissor lift department
1.2. Scissor lift /Boom lift Operator must be trained Supervisor,
1.3. Maintain inspection checklist. Safety person
1.4. Training Records must be submitted to M+W EHS Dept.
1.5. Scissor lift must be positioned on firm level ground and to be
operated only by a qualified operator
1.6. SWL signs to be displayed on the scissor lifts and safe
working loads shall be compiled to at all times.
1.7. Banks Man must be designated to guide the movements of the
scissor lifts at site Speed limit 12 Km/h
1.8. Ensure No hand phone usage except for emergencies.
1.9. Ensure access is firm and even.
1.10 Warning lights to be provided while night work
1.11. Ensure don’t move lift keeping height.
1.12.Ensure Swing alarm at boom lift
4. Drilling of 1.Electrical hazards 1.1. All electrical tools & equipments have to certify LEW before
Roof use. Ensure stickers are to be pasted on the equipment and
appliances. All inspected equipment or appliances are colour
coded.
1.2. To ensure extension cable are routed above 2 meter from
ground or hang on cable stand.
1.3. Workers to ensure when handling tools according to
manufactures instruction. All user must be briefed before use the
electrical tools.
1.4. To check on physical condition of cable to ensure it is free
from defect before use.
1.5. Ensure all electrical tools equipped with a dead man switch.
1.6. Always use ELCB of 30mA when connect to main power
supply.

starts. Kept all electrical tools in dry place ensure unplug from the

11. Gas cutting

3.3 Ensure before using the tip of torch shall be cleaned. EHS personnel,

4.1 Flammable/combustible materials will be removed from the work EHS personnel,
