Re: Hazard Identification and Assessment on Safety Practices on Vertical Structures in Cagayan de
Oro Construction Firms

We are writing to ask you to take part in an undergraduate research study, which aims to determine the
different hazards and safety practices adopted by the various firms in Cagayan de Oro City. A team of
researchers at the University of Science and Technology of Southern Philippines (USTP), Civil Engineering
Department is conducting this thesis study and the objectives of this research are as follows:

 To identify the hazards and safety issues in the selected sites of Cagayan de Oro.
 To determine where the identified hazards are most frequently found in terms of the vertical
construction.
 To determine the safety management practices adopted by the selected sites of Cagayan de Oro
through questionnaire survey.
 To find out how the adopted safety practices affect the duties and responsibilities of stakeholders
such as the contractor, the client and workers through interviews and questionnaire survey.

As part of this research we are conducting a survey, which is examining the hazards and safety practices of
small-scale, medium-scale and large-scale vertical construction projects in Cagayan de Oro. We would be
very grateful if you would return the enclosed questionnaire in the envelope provided together with your
letter of consent. We would be very happy to supply you with a copy of the results of the paper if you would
like them.
