This is with reference to the “kind of leave; holiday leave; sick leave;” that I am
entitled to as per our company policy.

I wish to take these said “name of leave” on 31 December 2018, 2 and 3 of

January 2019. The reason for taking these “name of leave” on these specific dates
is because me and wife had planned and scheduled the Baptism of our dearest
son SON SON on said dates. The baptism of our son would take place in my
wife’s province at Babatngon, Leyte/Tacloban City to give my in-laws a chance
to see their newest grandchild.

In connection with this, I would be able to resume my duties in our office on 4

January 2019.

I also attached in this letter a copy of our plane ticket and baptismal invitation
for your reference.

I am looking forward for the approval of my request.
