I would like to express my intention to apply for Housing Loan Restructuring and
Penalty Condonation Program. I commit to submit complete documentary requirements
within thirty (30) days from this Letter of Intent.

I am aware that the Pag-IBIG Fund will pursue the cancellation of the Contract to
Sell or initiate foreclosure proceedings, as the case may be, should I fail to comply with
the aforementioned requirements as I have committed.
