Age Appropriate Transition Assessments and Summary
Personal Interests:
Based on completed transition questionnaire, Jason enjoys student council and is currently
the treasurer. He likes to plays racquetball in a community league and enjoys watching
sports i.e. basketball, hockey, football on TV or in person. Jason is an active member of the
youth group at his church and enjoys hanging out with his girlfriend.

He wants to go to a 4-year state college and his career goal is to become a high school
teacher.
Training/Education:
-Jason will TA in a classroom on campus to shadow a teacher
-Grade papers
-Interview a high school teacher and make a road map for how he will achieve this goal
-Jason will graduate HS with the necessary requirements for a 4-year college
-Learn how to apply and write essay for college admission
-Complete career readiness class and on campus job internship
Employment:
-Jason spends time on the weekend working in his uncle’s legal firm where he answers the
telephone and files various documents.

Measurable Post-Secondary Goals
Training/Education Goal:
-Jason will identify 2 colleges/universities that he would like to attend (with education
majors)
-Jason will apply to attend a 4-year university education program
-Jason will meet with the disability office, provide his IEP and create a plan for
accommodations he may need in his college classes to be successful

Training/Education Connected IEP Annual Goal:
-Jason will be able to meet all requirements of a TA (teacher’s assistant) including grading
papers, taking attendance, making copies etc. as measured quarterly by teacher job review.
Employment Goal:
-Jason will obtain a job as a counselor at a summer camp where he is in charge of kids
- Jason may apply for a paraprofessional classroom aide for summer school
Employment Connected IEP Annual Goal:
-Jason will be able to meet all requirements of a a part time job of his choice measured
quarterly by job review completed by Jason’s employer and turned into case manager.
