Dear Madam,

This is to seek approval of the intention of Mr. Christian Cruz to enroll as regular student of
HUMSS 11, this second semester, school year 2018 – 2019. Mr. Christian Cruz failed his major
subjects during the first semester of this school year because of non – attendance. For
him to continue his studies, his mother, Mrs. Joycelyn Cruz made the following
arrangements with this office:

1. Mr. Cruz shall be allowed to enroll this second semester, 2018 -2019 as
a regular student as long as he also enrolls five (5) of his subjects in
modular classes.
2. The other three major subjects shall be taken during the first semesterof
2019 – 2020.
3. Modular classes shall be paid in cash.
4. In the event the Mr. Cruz fails to pass both his regular and modular
classes, he will be required to repeat the entire Grade 11 course.
5. Mr. Cruzʻs daily attendance shall be strictly monitored by the subject
teachers and the adviser.

Attached, for you perusal, is the schedule of his modular classes as arranged with the
teachers in – charge.
