(place of school), that I understood well the Hiring Guidance for T1 position.

a) CSC Form 212 ( Revised 2017) with latest passport size ID picture
b) Authenticated copy of PRC license
c) Certified photocopy of ratings obtained in the LET
d) Certified copy of Transcript of Records
e) Copies of Service Records, Performance Rating and School Clearance for those
with teaching experience. If unavailable, the applicant must submit a
justification citing the reason/s for unavailability
f) Certificate of specialized training, if any
g) NBI Clearance
h) Omnibus certification of authenticity and veracity of all documents submitted,
signed by the applicant
3. I am executing this Certification to attest to the authenticity and veracity of all the
documents submitted
4. I understand that any violation will automatically disqualify me from the selection
process
5. By executing this Omnibus Certification of Authenticity and Veracity of
Documents, I hereby authorize the Department of Education, SDO Cabanatuan through
the School Head, to verify the authenticity of the abovementioned documents.
