Notes / Narratives
On 3/1/19 at approximately 0225 hours, I was working as part of the Nighttime Traffic Enforcement unit operating a semi-marked
patrol vehicle (Shop 210509) and responded to 5401 N Scottsdale Rd reference a DUI investigation. I was in full SPD regulation
uniform wearing my AXON body camera.
Upon arrival, I spoke to Officer Vattuone #1493. Officer Vattuone stopped Cameron Maybin observing signs and symptoms of
possible alcohol consumption/impairment. I took over the investigation.
I contacted Cameron who was still seated in the driver`s seat requesting he exit the vehicle. Cameron stated he was coming from
Oceans 44 where he had 5 glasses of wine. After Oceans 44, Cameron went to his friend`s house and was now headed to Hilton
Villas.
While speaking with Cameron, I observed the following:
SPEECH: Clear
COLOR OF FACE: Apparently Normal
EYES: Bloodshot and watery
ODOR OF INTOXICATING LIQUOR: Yes
ADMISSION OF ALCOHOL CONSUMPTION: Yes
The area where the SFSTs were performed was flat, dry, and free of debris. The streetlights and my flashlight were used to
illuminate the ground. The emergency lights from my patrol vehicle were turned off so they were not in view of him during the tests.
The requested SFSTs were explained and demonstrated prior to Cameron beginning. The following results were recorded on the
Alcohol Influence Report (AIR).
HORIZONTAL GAZE NYSTAGMUS: 6 out of 6 Clues Vertical Nystagmus: No

After the 1 set of 9 steps, Cameron failed to turn around as he began to walk backwards. I discontinued the WAT test.

After completing the OLS, I asked how big the glasses of wine were. Cameron stated they were pretty big.

I asked Cameron if he thought he was above or below a .08%. Cameron stated he tried to sleep it off stating he slept at least an hour.
Cameron stated he hoped he was below a .08%
At 0243 hours, I placed Cameron under arrest for Driving Under the Influence of Alcohol. He was handcuffed to the rear and the
cuffs were double locked. I then secured him in the backseat of my patrol vehicle.
Per Arizona Revised Statute, the vehicle was impounded by All City Towing and a Vehicle Impound Supplement was completed. A
copy of the Vehicle Impound Supplement was placed in Cameron`s property and copies were mailed to all other registered owners
and/or lien holders.
At 0252 hours, I read Cameron the information from a standard Admin Per Se/Implied Consent Affidavit. Cameron stated "yes sir"
when asked if he would submit to the requested blood and breath tests. I transported Cameron to the District 2 Jail to conduct the
blood draw and breath test.

I inverted each tube 10 - 12 times and verified visually that there was a proper mixture between the preservative/anticoagulant and
the blood. I then properly sealed/labeled each vial and secured it in the blood collection kit. The blood collection kit was later
impounded in the secure evidence refrigerator at the District 2 Station.

Supplement Notes
On 03/01/2019, at approximately 0222 hours, I was on patrol in a fully marked SPD emergency vehicle, in full regulation SPD
uniform, wearing department issued On Body Camera. I was driving north bound on Scottsdale Rd when I observed a gray Range
Rover bearing North Carolina plate// DJR2032. I initially observed the vehicle at Scottsdale/ 3rd avenue traveling north bound.
During this time the Range Rover was speeding in a posted 35-mph zone. The vehicle, by my calibrated speedometer, was going
approximately 55-mph as I was directly behind the Range Rover approximately a full car length in distance. While continuing to
travel northbound on Scottsdale road in the #1 lane, just north of Stetson, the vehicle`s passenger side tires crossed over the #2 lane
line, thus failing to maintain a single lane.
From there the vehicle proceeded north bound on Scottsdale Rd, now in a 40-mph zone, going approximately 60-mph (paced). I then
initiated the traffic stop with my emergency lights turned on. The driver of the vehicle, later identified as Cameron K Maybin, began
to apply the breaks of the vehicle near Highland Ave. The vehicle did not come to a stop until we reached the entrance of Double
Tree Resort located at 5401 N Scottsdale Rd.
I then exited my vehicle and approached the driver side of the Range Rover. I contacted the driver who was identified as Cameron K
Maybin (via North Carolina DL). I asked Maybin if he had any idea why I was stopping him, Maybin advised that he did, but did not
specify as to why. I asked Maybin where he was coming from, he advised he was at dinner and coming from Ocean 44. I asked if he
had any alcohol this evening, he advised that he had a couple glasses of wine. I then began to tell him why I stopped him and
explained the civil traffic violations I observed. Maybin stated that he was in Arizona for Spring Training with the Giants and that he
was sorry for this. Maybin went on to say that he was going to meet his agent at the Scottsdale Villas.
Ofc. McCann arrived on scene and conducted a DUI investigation including the Walk and Turn and One Leg stand. Based on Ofc.
McCann`s investigation Maybin was placed under arrest. Maybin was polite and cooperative throughout the investigation.
FURTHER

At approximately 0243 hours, Ofc. McCann arrested Maybin for DUI. He was handcuffed to the rear per SPD policy; with handcuffs
check for fit before being double locked. Maybin was then escorted to the patrol vehicle where he was searched, incident to arrest
and secured.
Ofc. McCann completed his DUI investigation by completing the corresponding forms. Please refer to his supplement regarding his
investigation.
I remained on scene and completed the Vehicle inventory report (VIR). All City Towing arrived and removed the vehicle from the
roadway per ARS 28-872; Driver Arrested.
