JUDICIAL AFFIDAVIT OF DR. GLORIA MACAPAGAL ARROYO.

(For the Prosecution)

DR. GLORIA MACAPAGAL ARROYO, , offers her testimony by

way of Judicial Affidavit, to prove that:

1. The fact of death of DR. FERDINAND EDRALIN MARCOS;

2. The cause of death is a gunshot wound. The bullet entered through
left temple, passed through the frontal lobes of the brain, striking
and and fracturing the right temple area of the skull, but never
exiting the skull itself.

I, DR. GLORIA MACAPAGAL ARROYO, of legal age, Filipino, and

a resident of Digos City, Philippines, after having been duly sworn to in
accordance with law do hereby depose and state THAT:

PRELIMINARY STATEMENT

In accordance with A.M. No. 12-8-8-SC, I hereby execute this judicial

affidavit in a question and answer format. The person taking my testimony
is ATTY. CID BENEDICT PABALAN, with office address at 2nd Floor
Angeles Bldg., Camia Street, Cor. Dao Street, Digos City, and the taking of
my testimony is being held at the same address. I am answering his
questions fully conscious that I do so under oath and may face criminal
liability for false testimony and perjury.

DIRECT EXAMINATION OF THE EXPERT WITNESS

ATTY: (Formal offer of Testimonial Evidence): Your Honor, I would like to

offer the testimony of DR. GLORIA MACAPAGAL ARROYO, the
attending physician who preliminarily examined the body of the victim, DR.
FERDINAND EDRALIN MARCOS. SHe will testify as to the fact of death
of the victim. He will testify as to the fact of death of the victim, and that he
will testify that the victim died by a gunshot wound to the head.

PRELIMINARY QUESTIONS

Q: Before we begin, are you aware that you are under oath and may face
criminal liability for false testimony and perjury?

A: Yes, attorney.

Q: Please state your name and profession for the benefit of the court.

A: My name is DR. GLORIA MACAPAGAL ARROYO, a medical doctor

by profession.

Q: Kindly state your educational background leading to your profession as

of this date.

A: I obtained by Bachelor of Science in Biology Degree from the Ateneo de

Davao University in the year 1996. Thereafter, I went to medical school in
the Davao Medical School Foundation, Inc., graduated in 2001 and passed
the medical board exams in 2003.

Q: What is your current profession?

A: I am currentlys stationed at Dominican Hospital, Digos City.

Q: How long have you been practicing your profession as a medical doctor?

A: For about 15 years now, more or less.

Q: Is this your first time to testify in court?

A: No, this is not my first time.

Q: To the best of your knowledge how many times have you testified as an
expert witness?

A: I have made about 100 testimonies more or less, as an expert witness as

of this date.

Q: Having been stated these information about your years of experience as

a medical doctor and medico-legal officer, can you say with absolute
certainty that you are qualified to testify as an expert witness to attest the
cause of the death of DR. FERDINAND EDRALIN MARCOS?

A: Yes, I am.

DIRECT EXAMINATION PROPER

Q: Can you recall having attended to a patient in the name of DR.

FERDINAND EDRALIN MARCOS? far?

A: Yes.

Q: Where did you examine DR. FERDINAND EDRALIN MARCOS?

A: I examined the victim when he was rushed to Dominican Hospital.

Q: When did you examine the victim? Kindly recall the exact date and time
of the examination.

A: I attended to DR. FERDINAND EDRALIN MARCOS at around 6:00

o’clock in the evening of December 27, 2018

Q: Was that the first time for you to see DR. FERDINAND EDRALIN
MARCOS?

A: No. Actually he was a colleague of mine in the hospital where we worked.

Q: Can you describe the status of DR. FERDINAND EDRALIN MARCOS,
when you saw him?

A: Unfortunately, he was immediately declared dead upon arrival.

Q: Can you please discuss the examination you conducted on DR.

FERDINAND EDRALIN MARCOS?

A: I conducted a preliminary post-mortem examination of the body to

determine the cause of death.

Q: What were your findings?

A: The deceased suffered a serious head trauma, caused by an apparent

gunshot wound on his left temple. There is no evident exit wound though,
to my observation.

Q: Based on your findings, what in your best knowledge is the cause of death
of the victim?

A: The victim died most likely died of a gunshot wound to the head.

Q: Aside from the gunshot wound to the head, are there any other external
or internal findings that may have caused injury or death to the victim?

A: There are no other markings or injuries aside from the said gunshot
wound entering the victim’s left temple

Q: Do you remember preparing a document, narrating your findings on the

late DR. MARCOS?

A: Yes, I signed his Death Certificate.

Q: I am now showing you a copy of a Death Certificate prepared and signed

by one Dr. Gloria Macapagal-Arroyo, what is the relation of this report, from
the document that you stated earlier?

A: That is the document that I prepared, stating the fact of death of Dr.
Marcos.
Q: Do you affirm that you prepared, issued, and signed this Death
Certificate?

A: Yes.

Q: Do you confirm that the contents of this is true and correct and are you
willing to sign this as your conformity to all that has been stated in this
document?

A: Yes.

Q: I am attaching this Death Certificate as Annex “A” of your Judicial

Affidavit, are you confirming my action?

A: Yes.
