AFFIDAVIT OF DISCREPANCY

I, RUBEN CANATES MARUDO JR., of legal age, single, Filipino, and

resident of Hornalan, Calamba City, Laguna, after having been duly sworn to
in accordance with law hereby depose and state that:

My true and correct name is RUBEN CANATES MARUDO JR. born on

August 24, 1991 at Papandungin, Gloria, Oriental Mindoro. Copy of my
Certificate of Live Birth is hereto attached as Annex “A”;

The name RUBEN CANATES MARUDO was inadvertently registered

in my NBI Clearance instead of my true and correct name RUBEN CANATES
MARUDO JR.;

That RUBEN CANATES MARUDO JR. and RUBEN CANATES

MARUDO refer to one and the same person;

That this statement was made not to deceive any legal authority not to
evade any criminal and/or civil liability;

That I am executing this affidavit to attest to the truth of all the

foregoing facts and circumstances, to inform the proper authorities that my
true and correct name is RUBEN CANATES MARUDO JR. and for whatever
legal purpose this may serve.
