Greetings!

The Philippines has been a major source of human resources around the globe for decades. The
innate Filipino virtues of hardwork, perseverance, patience, easy adaptability to diverse cultures
and environments, among others, have made the Filipino worker a reliable, dependable and
loyal employee.

The oft-said saying “HEALTH IS WEALTH” cannot be more overemphasized to both companies
and employees alike. Healthy workers enable companies to provide better services and turnout
their products more efficiently with maximal profitability. Healthy employees prolong their
income-generating years to assure financial security for themselves and their families before
and after retirement. Preventative medicine and workplace safety are therefore important
practices to maintain everybody’s health and well-being.

EVERGREEN MEDICAL AND DIAGNOSTIC CENTER, an occupational medical clinic and primary
care clinic, serves to assist companies in their recruitment process by providing cost-efficient,
work place-appropriate, accurate and timely medical screening of their candidates. The strategic
location of the clinic along EDSA Pasay Metro Manila provides easy accessibility from all points

 LETTER FROM THE MEDICAL DIRECTOR

Greetings!

I have been practicing now for 12 years and I have observed the growing need for a high
quality-standard medical facility for OFW and Seafarers in the Philippines, and the potential
improvement the industry needs.

I worked as a cruise ship physician and underwent numerous pre-employment medical

examinations myself. I have experienced first-hand being a patient and undergoing through the
process of the complete medical examination. I also became the internal quality medical auditor
at a work clinic based in Washington USA. I was tasked to devise medical protocol based on
occupational safety and health administration imposed by the United Stated of America.

Equipped with these experiences, I was able to see the difference in the occupational medicine
between the Philippines and the US. As a physician and as an OFW-Seafarer, I aspire to apply
the valuable knowledge I have gained abroad, and to make a great impact in the occupational
health field. Together with my US-based physician partner, we put up Evergreen Medical and
Diagnostic Center. We envision providing high-quality medical services to ensure health,
wellness and productivity of the Filipino workers.

Introduction

EVERGREEN MEDICAL AND DIAGNOSTIC CENTER, INC. (EMDC) was conceptualized

in 2010. One of the founders of Evergreen Medical and Diagnostic Center worked as
a cruise ship doctor. She belonged to a family of seafarers, and had undergone the
required medical examinations prior to joining her assigned vessel. Working with
various crew members and passengers, she had a better understanding of the
common medical concerns and the importance of health and wellness at sea.

Partnering with U.S.-based doctor who provides primary care in the state of
Washington also known as the Evergreen State, Evergreen Medical and Diagnostic
Center Inc. was born. EMDC aims to provide excellent medical services to suit the
current needs of the Filipino worker – overseas foreign worker (OFW) and local
based.

 History

EVERGREEN MEDICAL AND DIAGNOSTIC CENTER, INC. (EMDC) was established as a

corporation under SEC in Nov. 4, 2013. The DOH Accreditation as a Medical Facility for
Overseas Workers and Seafarers in March 1, 2014. The DOH Accreditation for Drug Testing
Laboratory subsequently followed In March 28, 2014.

The Certification of Quality Standards System by Philippine Council on Accreditation of

Healthcare Organization (PCAHO) was obtained in August 16, 2014. Recognition by POEA,
MARINA, TESDA, PRC, Phil. Retirement Authority; Drug testing laboratory by LTO, Dangerous
Drug Board and PNP followed in September 2, 2014.

ISO 9001:2008, Bureau of Veritas was initiated in June 22, 2015 and was later acquired in
October 19, 2015.

The clinic floor area was expanded from 220 sq. m. to 550 sq. m. in late December 2015. This
gave way to adding the Evergreen heart station, a bigger laboratory, dental and psychometric
testing room.

In 2015, the accredition of various HMOs followed such as Intellicare, Cocolife, Valucare,
Philcare, HMI, Asianlife, HPPI, Avega, Getwell, Medicareplus, etc.

At present, EMDC is transitioning from ISO 9001:200o to ISO:2015.
