Guidelines for mechanical seal failure info checklist

1. Please fill up the date, time operator’s name and pump identification tag.

2. Specify the duty of the equipment and indicate any abnormal operating condition.

3. Indicate the seal operating temperature was normal or not and compare with pump’s

measurement.

4. Indicate whether vibration level is beyond normal.

5. Is cavitation or abnormal sound (example: squeaking from dry running) present?

6. Compare inlet and outlet temperature of seal cooler if fitted.

7. Specify the quench medium used (steam/wet) and pressure if used.

8. Specify the seal leakage pattern. Drips per minutes or spray/stream.

9. Is the leakage occurring during equipment stationary/running?

10. Specify which side of seal is leaking if it is double ended.

11. Specify where the leakage appears to come from.

12. Specify API piping plan (Plan 11/ 52/ etc.) of seal and if it should be pressurized or not.

13. Indicate the seal pot level is normal or not and the frequency of refilling.

14. Indicate whether vent port was opened or closed/plugged for unpressurized system.
