 TERMS:
i. Must initially purchase two (2) sets of reagents (Diluent, Rinse, Lyse and Cleaner) worth of Php 122,600.00.
ii. Starting on the 4th month of contract, purchase reagents (Diluent, Rinse, Lyse and Cleaner) worth Php
30,000.00 per month until the end of contract.
iii. 24 months term
iv. Upon finish of contract, the machine will be rewarded to the hospital/clinic as free machine provided that the 24
months monthly purchase of reagents was strictly followed.
v. During the 24 months, the supplier would warrant the machine.
vi. Warranty void if neglegence or damage by client or an unauthorized install of reagent/personnel.
vii. The machine is sole owner of the supplier until completion of full 24 months term of reagent purchase.

*Minimum of Php 200.00/ test (CBC and Platelet count)

*Operation per month (30 days)

10 patients x Php 200= Php 2,000.00/day

Php 2,000 x 30 days= Php 60,000/month

Total minimum patient tested per month: 300 patients

Cost per test: Php 44.88

Total cost of reagent: Php 13,464.00 for 300 patients

Php 60,000.000 – Php 13,464.00 = Php 46,536.00

Php 46,536.00 – Php 30,000.00 (monthly reagents) = Php 16,536.00 Total minimum net
