Please consider NICOLE ANN A. ALVISO, a Bachelor of Science in Business Administration

major in Financial Management student of the University of Cebu – Lapu-Lapu & Mandaue
(UC-LM), who wishes to apply as a Practicum in your prestigious establishment.

Ms. Alviso is officially enrolled in Practicum this Summer School Year 2018 - 2019, in
accordance with the existing curriculum for Bachelor of Science in Business Administration as
approved by the Commission on Higher Education (CHED).

We are confident that your good office has adequate resources and facilities that you may
provide to establish a dynamic linkage between the academe and business industry by giving the
students the opportunity to gain useful insights and worthwhile experiences in the actual work
environment through a realistic On-the-Job Training Program.

The following provisions, to wit, shall be conducted during the course of the On-the-Job
Training/Practicum of Ms. Alviso:

1. The training requires Three Hundred Sixty (360) hours of duty and the trainees
may be required to work for eight (8) hours during the regular working day but is
not limited to a preference of choice on their schedules;

2. Ms. Alviso shall undertake and perform on-the-job training activities related to
their job and academic requirements and not be assigned to undergo on-the-job
training beyond the Cebu Metropolitan area without prior notice to the University
of Cebu – LM, and that consent or authority must be obtained or secured from
their parents/guardian;
 3. That EVERSLEY CHILD SANITARIUM shall evaluate Ms. Alviso’s
performance during the course of the On-the-Job Training/Practicum based on the
Performance Evaluation Form (will be provided by UC-LM) and shall likewise
award a Certificate of Completion of the training program upon the termination of
Ms. Alviso’s tour of duty, for academic and completion purposes.

Attached please find Ms. Alviso’s application letter and waiver of undertaking for your easy
reference and perusal.
