license no. 34-26487-02, is the 3241- C.

Hello Ed,
Thank you for taking time out of your day to discuss your pending license amendment request. As we
discussed, due to changes to our licensing process Items 6-9 on your license will now appear differently going
forward. Specifically, instead of allowing a blanket amount of material to be split between multiple types of
devices, each device manufacturer and model will now have their own limit. To facilitate this change please
provide the following information:
a. Isotope Requested

b. Device Manufacturer
c. Device Model
d. Maximum activity for any single source.
e. Total activity requested for devices of this manufacturer and model.
f. For any of these devices if a specific sealed source is used please include the manufacturer and model
number.
For example, if you were to request to authorize two Troxler Electronic Laboratories Model 3400 Series gauges
I would at a minimum need to have the following information provided:
1. Cesium-137, Troxler Model 3400, 9 mCi per source, 18 mCi total
2. Americium-241, Troxler Model 3400, 44 mCi per source, 88 mCi total
