PCC-283 - FOCAL EPITHELIAL HYPERPLASIA: A CASE

Focal epithelial hyperplasia, or Heck’s disease, is a rare benign
viral infection of the oral mucosa caused by the subtypes 13 or 32 of
human papillomavirus (HPV). It is characterized by the occurrence
of single or multiple small papules or nodules in oral cavity, espe-
cially on the labial or buccal mucosa. A 35-year-old black male
sought dental care because of multiple lesions in the oral mucosa.
Intraoral clinical examination revealed multiple papular and nodular
lesions with an irregular surface, sometimes whitish, affecting the
labial mucosa and upper and lower alveolar ridge. An incisional
biopsy was performed in the region of the lower lip and alveolar
ridge, and the histopathological diagnosis was focal epithelial hy-
perplasia. Although HPV-induced manifestations have similar ef-
fects, the correct diagnosis and management of these lesions ensures
the patient a better prognosis and therapeutic indication.
PCC-284 - GINGIVAL METASTASIS FROM COLON
ADENOCARCINOMA: A CASE REPORT. JEANNE DA

Oral metastatic tumors are uncommon lesions, and the attached
gingiva is the most commonly affected oral soft tissue site. The mean
age is 65 years, and the mean survival is 9 months. The most
common primary sites of metastatic tumors are the lung for men and
the breast for women. An 89-year-old white female presented with a
painless mass in the upper gingiva that had evolved over the course
of 15 days. The medical history included colon cancer 2 years earlier,
with bone metastasis in the iliac crest, clavicle, and hip. Clinical
examination revealed an extensive nodule on the attached gingiva of
the right upper canine. With the provisional diagnosis of metastatic
tumor, an incisional biopsy and immunohistochemical examination
confirmed the diagnosis of metastatic intestinal adenocarcinoma.
The patient died 4 months later. Despite their rarity, such lesions
should be considered in the differential diagnosis of inflammatory,
reactive lesions with rampant evolution.

The gubernacular cord, located within the gubernacular canal,
links the tooth follicle to the overlying gingiva and can be identified
in the alveolar region of the lingual/palatal surface of deciduous
teeth. Some authors believe that the gubernacular canal and cord
play a role in the guiding the course of tooth eruption, whereas
others claim that these structures can influence the development of
an adenomatoid odontogenic tumor (AOT). Here, we present the
case of an 11-year-old female who, on a routine panoramic
radiograph, presented a well-defined radiolucency around the
crown of an impacted tooth associated with a canal in the apical
direction, which was diagnosed as hyperplastic dental follicle

associated with gubernacular cord. It is likely that most cases
presenting gubernaculum dentis are undiagnosed or misdiagnosed.
Current data show that there is a histogenetic relationship between
the gubernaculum dentis and AOT; hence its importance.

Herpes zoster (HZ) is an acute viral infection caused by reac-
tivation of varicella zoster virus (VZV). Following the primary
varicella infection, VZV establishes latency in dorsal root or cranial
nerve ganglia. Reactivation of VZV can result in its spread from the
ganglion to the corresponding dermatomes, producing neuro-
cutaneous signs and symptoms. A 76-year-old female reported
having severe pain and a burning sensation on the right side of her
face for 5 days. The clinical examination revealed diffuse edema,
widespread erythema, and vesicles distributed over the right
ophthalmic, maxillary, and mandibular divisions of the trigeminal
nerve, exhibiting unilateral involvement. Intraorally, vesicular
eruptions, erythema and areas of ulcerations were noted unilaterally,
including the hard palate and buccal mucosa. The diagnosis of HZ
was confirmed by blood tests, and the patient was treated with
acyclovir, analgesics, and vitamin B. By approximately 2 weeks
after the initial presentation, the lesions had cleared completely.
PCC-287 - HIV-INFECTION DISCLOSED BY ORAL AND
CUTANEOUS HISTOPLASMOSIS. ISADORA PERES

Histoplasmosis is an endemic, systemic mycosis caused by the
fungus Histoplasma capsulatum. Disseminated histoplasmosis is
usually an opportunistic disease, affecting immunocompromised
patients, that spreads to multiple organs. Cutaneous manifestations
occur in 38e85% of patients and oral lesions in 30e60%. A 32-
year-old female was referred for evaluation of an extensive ulcer,
on the tongue, that had grown for a month without any treatment.
The patient also presented angular cheilitis and an undiagnosed
ulcerative skin lesion on the breast. The diagnostic hypothesis was
squamous cell carcinoma. Examination of an incisional biopsy
specimen revealed granulomatous inflammation, suggestive of
histoplasmosis. Considering the features of the disease, hemato-
logical tests were requested and indicated positivity for HIV. The
patient was treated with ofloxacin (400 mg, 12/12 hours, 6 months)
by the physician. After 5 months, complete healing was observed,
and the patient remained in continuous care for HIV control.
