(APPORTIONING BLAME)
First Stage

1. Reference is made to HQCAA letter No. HQCAA/2388/511/HR(Inq)/574 dated 09th October,


2018.
2. At this stage, it appears to the Board that you Engr. Sumair Saeed (Ex- Director HR) Witness
No-8 is likely to be blamed for the following;
a. For failing to hire permanent operation & maintenance staff for operation and
maintenance of sensitive equipment like PBBs prior to operationalization of IIAP
Islamabad.
b. For failing to take up the matter to competent authority for approval of permanent staff
(as exception) for sensitive equipment like PBB having tremendous repercussion in
respect of cost and image of organization.
c. For failing to materialize the operation & maintenance contract for PBBs prior to
operationalization of IIAP Islamabad because of which manpower could not be trained
properly prior to operations.
d. For taking 39 unskilled manpower from a contractor having contract agreement for
Operation & Maintenance of E/M facilities other than PBBs. These personnel were taken
in respect of E/M facilities but deployed in PBBs.
3. You are informed accordingly and also apprised of your privileges under clauses (i) to (v) of
Para 19.2 of CAAO 5-5 (revised). Kindly submit your response if any by Date positively

2. At this stage, it appears to the Board that you Engr. Rabia Salma (Ex- Director Engineering
Services)) Witness No-26 is likely to be blamed for the following;
a. For non-provision of skilled / trained manpower who attended Package-09 training on
permanent basis at IIAP Islamabad.
b. For poor planning of deployment of maintenance staff for PBBs prior to
operationalization.
c. For taking 39 unskilled manpower from a contractor having contract agreement for
Operation & Maintenance of E/M facilities other than PBBs. These personnel were taken
in respect of E/M facilities but deployed in PBBs.
3. You are informed accordingly and also apprised of your privileges under clauses (i) to (v) of

Witness No-10 is likely to be blamed for the following;
a. For failing to hire permanent operation & maintenance staff for operation and
maintenance of sensitive equipment like PBBs prior to operationalization of IIAP
Islamabad.
b. For failing to take up the matter to competent authority for approval of permanent staff
(as exception) for sensitive equipment like PBB having tremendous repercussion in
respect of cost and image of organization.

2. At this stage, it appears to the Board that you Engr. Muhammad Rashid (System Engineer –
In-Charge PBB Maintenance) Witness No-19 is likely to be blamed for the following;
a. For negligence, overlooking of sensitive issues, taking the important issues lightly &
casually. Since the rotunda pin issue highlighted well before the incident being M/S
ADELTE trained engineer hired by CAA for the said job, no counter measures were
taken to prevent failure till permanent resolution of the issue.
b. For non-usage of proper tools to ensure proper torque on the fixing bolt.
c. For non-implementation of SOP for Operations & Maintenance of PBBs in letter & Spirit.

2. At this stage, it appears to the Board that you Engr. Mirza Muhammad Aleem (Joint Director
Mechanical – In-Charge of PBBs) Witness No-13 is likely to be blamed for the following;
a. For negligence, overlooking of sensitive issues, taking the important issues lightly &
casually. Since the rotunda pin issue highlighted well before the incident and no counter
measures were taken to prevent failure till permanent resolution of the issue.
b. Poor supervision & guidance of manpower working under him i.e. PBB maintenance
team and PBB operators.
c. For failing to communicate the serious operational incidents of PBB first collision to
higher authorities and not highlighting of incidents in IOU.
d. For non-implementation of SOP of Operations & Maintenance of PBBs in letter & Spirit.
