AT ISLAMABAD INT’L AIRPORT DATED 09-10-2018

1. PAF Team comprising of under mentioned officers have been detailed with effect from
01 December, 2018 as co-opted members to probe into circumstances under which PBB No 5
at IIAP collapsed vide AHQ/12334/16/343/Mov/PB dated 30 Nov, 2018.

2. The subject PAF team contribute immensely to the undergoing BoI. Their contributions

The above-mentioned officers remained attached with BoI from 01 December till 12 December, 2018 at
CAA, IIAP Fateh Jhang.

First Stage

1. Reference is made to HQCAA letter No. HQCAA/2388/511/HR(Inq)/574 dated 09th October,


2018.
2. At this stage, it appears to the Board that you Mr. Fawad Khan (M/s ADELTE JV) Witness No-
12 is likely to be blamed for the following;
a. For not analyzing failure mode and not providing appropriate mitigation measure in
design to prevent catastrophic failure of rotunda pin assembly.
b. For not taking any countermeasure to prevent catastrophic failure after having known the
rotunda pin dislocation issue.
c. For not providing proper maintenance training as only class training for how to use O&M
manual was provided.
d. For shortcomings in the O&M Manual regarding the elaboration of maintenance
procedures of critical parts / assemblies
e. Poor response in rectification of design & installation defects and shifting of responsibility
to operations team.
3. You are informed accordingly and also apprised of your privileges under clauses (i) to (v) of
para 19.2 of CAAO 5-5 (revised). Kindly submit your response if any by Date positively

1. Reference is made to HQCAA letter No. HQCAA/2388/511/HR(Inq)/574 dated 09th October,


2018.

2. The competent Authority i.e. DGCAA has convened a Board of Inquiry (BOI) on the above
subject. The copy of the terms of reference (TOR) is attached herewith. The Board needs your
statement as ex-project director IIAP Islamabad to proceed further. Kindly appear before the BOI on 8th
November 2018.
