INSTRUCTIONS
Note:- IN THE FOLLOWING PARAGRAPHS”BOARD” or “Board
of Inquiry’ is to be interpreted as including “Investigating Proceedings OF BOARD OF INQUIRY TO PROBE INTO THE REASONS OF
Officer”.
Instructions For The Board COLLAPSE OF BOARDING BRIDGE (PBB) AT IIAP ISLAMABAD DATED 09-
1. Foolscap, single plain sheet of paper to be used for 10-2018
recording evidence. Only one side of paper to be used.
2. Full signature of each witness and date must be inserted
at the foot of each page of his evidence and at end of his Assembled at ISLAMABAD INT’L AIRPORT
evidence, as close to the last line as possible.
3. All alternations and erasure in the record of evidence
must be initiated by the witnesses. On 11TH OCTOBER 2018
4. Provisions of paras 49 & 50 of PCAAO 5-5 are to be
complied with whenever the evidence reflects adversely on
the charter or professional reputation of a PCAA employee. By the order ofDIRECTOR HUMAN RESOURCE HQPCAA
In case of the pensions also similar privileges are to be given.
5. When the proceedings are completed they are to be
arranged in the following order by the Board before For the purpose of TO PROBE INTO THE REASONS OF COLLAPSE OF
submitting them to the assembling authority:- BOARDING BRIDGE (PBB) AT IIAP ISLAMABAD
(a) Pages (i) & (ii) of this Form are to be completed
as to assembling Authority understand, terms
of reference, composition of the Board, officers
in attendance, if any particulars of witnesses
and list of Exhibits.

(b) Diary of action. Showing day-to-day progress of
the Board, reasons for delay, if any, are to be
indicated therein. (To be recorded on page No

numerals as 1,2,3, etc. Check that the
provisions of para 49 & 50 of PCAAO 5-5 have
been complied with, where applicable.

6. It is the responsibility of the Assembling Authority to
ensure that the proceedings including their copies are
complete in all respects before these are forwarded to
higher authorities.
7. Remarks by the APM and the ZM/Director where IN ATTENDANCE
applicable should be entered in the appropriate column of
this Form. The remarks should include a general expression
of agreement or otherwise with the findings and
recommendation of the Board and specific recommendation
as to disciplinary action, write-off action etc.
