Ltd. and Mis Siddhi Enterprises (hereinafter referred to as the "Service Provider” which expression shall, unless repugnant to the context or meaning thereof, include its successors and permitted assigns and substitutes) of the Second Part. & WHEREAS. a the Authority had invited proposals vide Request for Proposal Document bearing No. SPMU/NHM/Procure/CT Scan/2016-17/27/01A dated 20/10/2016 (the "Bid Notice"}from. 4 Service Providers for provisioning of CT Scanner and associated ‘equipment, Installation, Commissioning and Maintenance of CT Scan facility including CT Scanner and all associated {1 cquipment, Operations and Management of CT Sean Services with skilled and unskilled manpower complying to AERB requirements for the locations in the State of Uttar Pradesh, i Rates in Schedule A of this Agreement; ‘or Star h & Path Lab Pvt Ltd, aa Director iContract Agreement for CT Scan (Category 1) Service Provider for Select Locations in Uttar Pradesh B.
