I, JUAN T. BERNAL JR., Filipino, of legal age, married and with residence
address at Obo, San Miguel, Catanduanes, after having sworn on oath in accordance
with law, hereby deposes and says:

1. That I was elected as a Barangay Kagawad of Obo, San Miguel,

Catanduanes during the 2013 barangay elections.

2. That I was not employed in any other government position or capacity

during my incumbency from November 2013 to November 30, 2016.

3. That during my term in the said position, other than my honorarium as a

Barangay kagawad, I have not received any other emoluments in the
form of salary/compensation from the government.
4. That I am executing this affidavit to attest to the above facts and for all
legal intents and purposes, it may serve.
