Formative Assessments

Learning objectives:
 English Language Arts- When shown sentences on the SmartBoard, TSW work in
groups to match the illustrations with the sentences by listening to the words used in the
sentence.
 Social Studies- When given materials, TSW create the American flag by gluing strips of
red and white paper and placing stars in the top left corner of the flag.
 Science- After exploring, observing, and creating vibrations, TSW describe the centers
by drawing how the vibrations occurred.
 Math- When given an exit slip, TSW identify the numerals by drawing circles to
represent the numerals.
Description of Assessment:
 English Language Arts- A formative assessment will be used by having the students
give a thumbs up if illustrations match sentences or a thumbs down if they do not match.
A checklist will be used to collect data and mark if students are correctly matching
illustrations to sentences.
 Social Studies- The formative assessment used is a child-friendly rubric. The rubric
assesses the students on three criteria: putting name on paper, gluing papers in correct
order, and putting silver star stickers in the top left corner.
 Science- An exit slip will be used as a formative assessment. The students will draw one
vibration center on the exit slip that they observed and created vibrations in.
 Math- An exit slip will be used as a formative assessment. The student will read the
numerals on the exit slip and draw circles to represent the numeral.
Rationale for Assessment:
 English Language Arts- The checklist will help the teacher candidate to determine
which students are able to correctly match illustrations to sentences and who are not able
to do so yet. Additionally, having students give a thumbs up or down can alleviate
pressure and nervousness to answer verbally.
 Social Studies- The child-friendly rubric will allow students to reflect on their work and
assess what they have produced. The teacher candidate will read sentences to the students
who are unable to do so.
 Science- The students will think about what they did at the vibration centers to cause the
vibrations and pick one vibration center to draw and demonstrate. This form of formative
assessment will allow students to draw their understanding versus writing. There are
eighteen out of twenty-five students who are struggling learners and having difficulty
writing.
 Math- This assessment will allow students who are struggling to identify the numeral
and draw circles. Prior to giving this exit slip, the students will have prior experience
drawing circles on the SmartBoard to represent the numeral with the teacher candidate.
