    CERTIFICATION

    This is to certify that one (1) Ely L. Ramos, 22 years old, house keeper, single,
    resident of 205 2nd street 6th avenue, Caloocan City personally appeared to this office to
    put into record the incident on March 27, 2019 at about 3:30 AM at Footbridge
    Monumento near Puregold.
    On about cited TDPO complainant claimed that while walking at footbridge when
    suspects with intent to gain forcibly took and carted away and fled to unknown direction.
    The stolen items are bag (Blue) that contains uniform, wallet that contains
    company ID, Philhealth, Tin ID, and more or less two thousand pesos.

    Disposition: For follow up for possible arrest of suspect.
