 Names, nationalities and residences of incorporator/s
 Authorized capital stock and shares with and without par value
 Name and nationality of the subscribers of shares of the authorized capital

 Number of shares subscribed, amount subscribed and amount paid by the
subscribers mentioned above
 Name and signature of the Treasurer
 Name and signature of the incorporator/s
 Is the corporation to be formed shall have perpetual existence? If not, indicate
