DEED OF QUITCLAIM AND WAIVER

Know all men by these presents:

This Deed made and executed by:

________________ of legal age, Filipino, single/married with residence and postal address at
_________________ Philippines herein referred as the “FIRST PARTY”;

____________________ with business address at ___________________________
represented by ___________________ herein referred as the “SECOND PARTY”;

WHEREAS, the FIRST PARTY was dismissed by the SECOND PARTY from employment as

WHEREAS, in consideration for the payment by the “SECOND PARTY” to the FIRST PARTY” of
the amounts indicated below, the FIRST PARTY”, his heirs, successor and assigns has agreed to keep
the “SECOND PARTY” free and harmless from any claim whatsoever arising out of or in connection with
the FIRST PARTY’s employment from “SECOND PARTY”

NOW THEREFORE, for and in consideration of the foregoing, the FIRST PARTY” hereby agrees
as follows:

1. The FIRST PARTY fully and comprehensively releases, forgives, quitclaims and discharges the SECOND
PARTY, its successors and assigns, and by these presents does hereby release, forgives, quitclaim and
discharge the SECOND PARTY, successors and assigns, from claims, demands, obligations, liabilities,
indebtedness, causes of action and expenses for every type, kind, nature, description or character, whether
known or unknown, real or contingent, in connection with or arising out of the FIRST PARTY’s employment
FROM THE “SECOND PARTY”.
2. The FIRST PARTY hereby acknowledges that the amount paid to him by the SECOND PARTY upon his
termination of employment is the whole and complete settlement of any and all amounts due and payable by
the SECOND PARTY to the FIRST PARTY in connection with or arising out of the FIRST PARTY’s
employment. Accordingly, the FIRST PARTY likewise waives in favor of the SECOND PARTY any present
or future claims, demands, obligations, liabilities, indebtedness, present or future claims, demands,
obligations, liabilities, indebtedness, causes of actions and expenses of every type, kind, nature, description
or character, whether known or unknown, real or contingent, in connection with or arising out of the FIRST
PARTY’s employment.
3. In executing this Deed, the FIRST PARTY hereby acknowledges that he has not relied upon any
representation of nor any representation has been made by the SECOND PARTY.
4. The FIRST PARTY shall fully and comprehensively compensate the SECOND PARTY for any and all costs
and expenses which the SECOND PARTY may incur by reason of any claim or action filed by the person of
the FIRST PARTY against the SECOND PARTY arising out or in connection with this Deed of Quitclaim and
Waiver.
5. That I acknowledge having received the total amount of _________________ (Php ___________) PESOS
ONLY paid to me in cash, in full and final settlement of any and all claims of whatever kind and nature which
I may have against ____________________________.
6. This release is an irrevocable and absolute waiver of and a complete bar to any pending action or
proceeding or those which might otherwise be brought by me or my heirs, successors, assigns and privies in
the future in whatever nature, whether in law or in equity against __________________________________
his agents, successors, assigns and privies. Should any provision be found invalid, void or unenforceable
the remaining provisions shall remain in full force and effect.
7. That I hereby declare that I have read and fully understood this document and/ or that the same was read
and fully explained to me before signing it and that this Release, Waiver and Quitclaim is made willingly and
voluntarily by me and with full knowledge of my rights under the law.
