Date Description and purpose of contact

1-10-2019 I called Mrs. Carter to see if it was a convenient time to talk and learn a little bit more
about her family and Anna. She asked me if I could call her back tomorrow (1-11-2019)
because she was expecting her children to come home from school, and didn’t think
she would have enough time to give me all the information that I would need before
they got home. She said that I can call her tomorrow at 11:15 a.m.

1-11-2019 I called Mrs. Carter to fill out the child information sheet so my partner and I could have
some information on Anna and her family before we went into the home for the first
home visit. Mrs. Carter was able to answer the questions I asked her, and I was able to
type her answers as she and I talked.

1-16-2019 Mrs. Carter texted asking if we could move our appointment tonight from 6:45 to 7:30
due to Parent Teacher conferences.

1-16-2019 Caili and I went to the Carter home to do our first home visit at 7:30. We got the
permission forms signed, and performed the RBI interview with Mrs. Carter.

1-22-2019 I texted Mrs. Carter to confirm the home visit for tomorrow night (1-23-19) at 6:45 p.m.
Mrs. Carter replied saying that time still worked for her.

1-23-2019 We went to the Carter home for our home visit at 6:45. We began to administer the
Vineland-II and we also watched videos on how Anna uses the switches. With the
videos, we were able to get a better understanding of how we can best interact with
Anna.

1-24-2019 As we were reviewing the Vineland-II, I noticed that I did not ask questions regarding
certain parts of the test that would apply to Anna. I texted Mrs. Carter to ask her if
there was a convenient time to call to ask a few questions, or if it would work better to
finish the questions at our next home visit next Wednesday. I was able to call Mrs.
Carter at 12 and finish asking the questions that I had missed.

1-30-2019 We were able to go to the Carter’s home to do the trial intervention session at 6:45
p.m.

2-4-2019 I contacted Mrs. Carter to confirm our observation of Anna for Wednesday February 6.
I also verified to see if we could do our IEP Meeting next Wednesday February 13 at her
home. Mrs. Carter said both dates were fine, and wondered if we were planning on
working with Anna after we observed her on Wednesday.
2-5-2019 I contacted Mrs. Carter asking if she could send an address of the Occupational
Therapist.

2-6-2019 Mrs. Carter texted me informing me that one of her children was sick, so she and Anna
might be a little late to the Occupational Therapy session. I let her know that was fine,
and later let her know that we were able to find the building.

2-14-2019 Brother Christopherson asked me to ask Mrs. Carter if he could bring about 5 faculty
members to our intervention session next Wednesday so they could see Assistive
Technology in action. Mrs. Carter said that would be fine.

2-20-2019 Brother Christopherson emailed me saying he would need to take a raincheck for
coming to see us work with Anna (along with the other faculty members). I texted Mrs.
Carter to let her know of the change of plans, and she messaged me back giving my
permission to send her contact information to Brother Christopherson so they could
schedule an appointment to see different AT with Anna.

2-20-2019 Intervention Session at the Carter home- 6:45.

3-6-2019 Intervention session at the Carter home-7:00 p.m.

3-13-2019 Intervention session at the Carter home- 7:00 p.m.

3-20-2019 I texted Mrs. Carter to confirm the time of our home visit. She needed to move it back
a little so we began the intervention session in their home at 8:00 p.m.

3-27-2019 I texted Mrs. Carter to let her know that we were coming, but might be a few minutes
late due to technical difficulties with printing a picture for one of the activities we were
going to do.

At 7 p.m. we began our intervention session.
