1. Description of activities you engaged in this period.


I was getting to know the office staff and create a general idea of what they were doing in
the Health Management Information office. In addition, I spoke with Information Technology
and Human Resources. In addition, I took a training where I learned how to work with Document
Capture System (DCS). Finally, I was engaged with the translation of a clean hand flyer for the
hospital.

a) Describe your activities in some detail.


Some of the activities that I performed during this period of time include the knowledge
of the office’s staff, what the staff was doing in the HIM office which is composed of the
management of the hospitals’ medical documents from the patients. In addition, I worked with
the Document Capture System (DCS) in order to work with a project that Ms. Filipovich has
assigned to me. The project consists of ordering and reviewing the cardiac monitor strips in order
to make sure that it was a high-quality document. Once I was done with the reviewing of the
cardiac monitor strips, I started to scan all those documents in order to transfer the images to the
Crouse information system. In addition, Ms. Brutcher, the Supervisor of Customer Service,
asked me for the favor of translating to Spanish a clean hand flyer. The purpose of these flyers
were in order to motive and encourage everyone in the hospital of constantly cleaning and
washing their hands in order to avoid the spreading of the diseases among the hospital and the
individuals working in it and/or staying in the facility.

b) Include your reactions to these activities and to your clients and peers.
My reactions were very professional, but internally I was so happy for all the process that
I was watching and involved in; it was very exciting to actually see what the real world of my
major was like. This week was more of getting to know the HIM and Customer Services
departments, and understand the staff’s daily tasks to prepare for what I was going to be working
with and the work I was going to be looking at.
c) Were you supervised during these activities?
Yes, I was supervised during these activities. The majority of the time Ms. Squadrito was
with me, but if I was in an employee’s office or in my office, then I was without supervision.

1. If yes, describe the supervision and include your reaction to your supervisor
and supervision.
The supervision was very professional, and, at the same time, I felt their
supervision was great since they always made me feel so welcome. My reaction to
Ms. Squadrito supervision was professional and very friendly and always made
sure I was understanding everything.

2. If no, describe your activities when you were not supervised and your
reactions.
When I was without supervision, I had the same behavior as if I was being
supervised because I wanted to make sure everything I did was done effectively
and responsibly. I had a great respect for Ms. Filipovich and it was never my
intention to lose the trust and faith that she gave to me.

2. What did you expect to learn through these activities?

I expect to learn more in depth whether the office’s daily work, purpose and
Mission, are true according to the actions that the staff perform and the services that
they provide.
b. Other “professionals” with whom you had contact?
I contacted the Customer Services Office, and the Information Technology Office.
Furthermore, I met Ms. Shepard's staff which constantly helped her with her role in
the hospital and my expectations, which were to learn more about other professionals
and see their everyday tasks, were met.
c. Your clients?
I did not have any direct contact with the clients.

3. What exactly did you learn through these activities?

I learned in a lot of depth the reason why this organization is a “non-for-profit”
one and how it really is engaged with employees, doctors, patients, among others in
all aspects. I also learned what “non-for-profit” really means and how Crouse
Hospital is so committed with the community’s needs and resources and with the
citizens that come to obtain healthcare services.

4. Can you identify any initiatives as required by the organization’s performance improvement
planning or as required by the regulatory bodies, such as CMS?
a) If so, what are they and who is requiring them?
Yes, I can. Ms. Filipovich office is one of the offices that was in charge of
regarding some of the obligations as an intern; I was in charge of manage and deliver
patients medical information. I helped Ms. Filipovich in these activities in order to
improve the performance plan of the information management of Crouse’s patients and
help the team to keep the medical information up to date and organized in a more
effective way.

b) If you can't identify do you see an opportunity for performance improvement?

5. What activities do you expect to be engaged in during the next 40 hours of internship work?
I am expected to be engaged in the training of the DCS system in order to make sure that
I knew all the steps and avoid any errors during the process of my work.

6. What do you expect to learn during the activities of the next 40 hours of internship work?
I am expected to learn how to send the documents to the right folders after I scan all the
documents that Ms. Squadrito are going to assign to me. In addition, I expect to learn more in
depth more about the department I will be working in and make evident the expectations of the
job to help the team effectively and efficiently in every manner. Furthermore, I expect to learn
and get to know more of the staff in the hospital and how they perform their jobs daily.

7. What do you WANT to learn? Have you communicated this to your supervisor?
I want to learn how to avoid errors during the process of upload the patients documents to
the computer system. Yes, I have communicated this to my supervisor.
