TRAINING CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

I, _______________________ of legal age, single , residing at _______________________

____________________________________________, do hereby agree to undergo TRAINING in the
______(COMPANY) under the following terms and conditions.

1. That I shall undergo TRAINING for 6 months in the ____(COMPANY) effective ____(START DATE) the
nature of which I am fully aware of is only training in nature.

2. That I shall diligently perform the duties pertaining to the job for which I am training for and such other duties as
my Employer may assign to me from time to time, and that I shall faithfully comply with all the rules and regulations,
performance, work and quality standards of the company.

3. That it is understood that I shall be training on a fixed-period basis starting ____(START DATE) until
____(END DATE) .

4. That I am medically and physically fit to undertake the Training.

5. That it is clearly understood that during the training period, I shall undergo 3 evaluations: on the end of first
month, on the end of the third month and on the end of the sixth month. And if, at any time, within the period
aforementioned, I am found unqualified or unfit for this particular training, the company has the prerogative to end
my training under this contract.

6. That the company shall have no obligation whatsoever to convert my training into a regular employment either
before or after the expiration of this Agreement.

7. That my training rate shall be Three Hundred Ninety Pesos (Php 390.00) / (or any amount that we will
be giving) per day with free duty meals. I am also entitled to Over Time pay, subject to approval by supervising
officer and/or store manager and shall get one day off per week.

8. That my training shall automatically expire on ____(END DATE) .

9. I agree that all records and documents of ______(COMPANY) and all information pertaining to its
business or affairs are confidential, and that no authorized discussion of the same will be made by me at any time
during or after my employment by the company. I will be held liable for any unauthorized disclosure of any
information I may acquire by virtue of my employment. I further agree that all records, documents and assets in my
custody or control shall be immediately surrendered to the ______(COMPANY) during the training
period, and at the termination thereof, whether or not requested.

10. That I hereby acknowledge to have read and understood the terms and conditions of my training as stated above.
