About This Document

Purpose and Scope

Purpose
This document comprises the administration guide for ViTALConnect 4.1, the latest release of the Vital Images’
medical diagnositc tool that allows physicians to use PCs or notebook computers to access 2D, 3D and 4D
advanced visualization using a Microsoft Web-enabled browser. The goal of the software is to enable users to
measure, rotate, analyze and segment images. Its conferencing feature allows physicians at multiple locations
to consult while simultaneously interacting with a study.

Scope
The scope of the document is to provide a detailed description of the configuration processes related to
ViTALConnect 4.1.5. Refer to the ViTALConnect 4.1.1 Installation Guide for a detailed description of the
ViTALConnect 4.1.1 installation process.

Intended Audience
This document is intended for experienced administrators who wish to configure a ViTALConnect 4.1.5 server on
Windows, or one of its variants. The authors assume that the audience knows how to perform standard
administrative tasks and are also familiar with the ViTALConnect system architecture, components, and
concepts. As such, this document covers ViTALConnect 4.1.5 configuration on Windows operating systems only.

Before You Begin


CAUTION: For general ViTALConnect Safety and Regulatory Considerations, refer to the Safety and
Regulatory Considerations section of the ViTALConnect User Guide.

NOTE: The ViTALConnect server must have been previously installed before you can use this guide.

Before you begin configuring the ViTALConnect system, you need to know the following information and have
the following materials:

• System Administration user name and password


• Knowledge of the institution’s server and network environment
• Remote DICOM server information
• SMTP server configuration
• Certificate file and Key file (for Secure Sockets Layer)
• Email addresses for monitoring and support destinations
• ViTALConnect License key (supplied by Vital Images)
• Load Balanced system topography

Configuring the System


Once you have installed the server software, you must perform additional steps to configure the system for your
specific network, DICOM devices, IP addresses, ports, user accounts and miscellaneous other components.

ViTALConnect system configuration consists of the following components:

• Configuring ECF (DICOM server)


• Configuring the Image Server
• Configuring user accounts
• Configuring user logging and reporting
• Configuring load balancing

NOTE: Every time you change the DICOM server configuration, you must open the Services Control Panel and
restart the “VICommunicator” or “VIServices” Service or reboot the server for the changes to take effect.

Accessing the Server Management Pages


You access the administration pages through the ViTALConnect Server Management System menu.

NOTE: Start from this menu to access all sub-menus described in this manual.

To access the ViTALConnect Server Management System menu:


1. In the Address line of Internet Explorer, type “/main.aspx” after the URL that accesses the ViTALConnect server (for
example: “http://172.29.8.200/main.aspx”) or from the Start menu, select Programs | ViTALConnect | ViTALConnect
Server.
2. Click GO.
The ViTALConnect Server Management System login screen displays.
3. Log in, using the following login and password combination:

Once you have installed both the DICOM server and the image server, and configured the DICOM server, you
must also configure the image server. Configuration includes a few additional steps for the following

Basic Settings

To configure basic Image Server settings (Main Page):


• Complete the entries as follows:

ViTALConnect Server Verify the default 8888.


Port
NOTE: In a load balanced system, each port must be different. Vital Images
recommends numbering the ports sequentially, starting with 8888, for the Main
server, then 8889, 8890, etc. for the Load Balance nodes.
Maximum of Studies Leave blank, or enter a maximum number of studies a user can open during one
user can open session. Fewer studies open leaves more system memory available, reducing the
possibility of slow performance due to low memory resources.

Referring Physician • Checked: Referring physicians can view only images associated with their own
Match patients.
• Clear: referring physicians can view all images for all patients.
Enable mark study • Checked: Workflow features such as mark read are enabled, and the resulting
as read study status (Scanned, Dictated, Approved) will be listed on the Study Directory.
• Clear: the Mark Read menu options are not visible, nor is the Status column in
the Study Directory.

NOTE: See ViTALConnect online Help for more information about these features.

Server process auto The amount of time the server can remain idle before an automatic process restarts
restart time interval it. Accept the default interval of 1 Days, 0 Hours, and 0 Minutes, or enter a different
time interval.

Built-in Account for This provides an alternate method of authenticating URL integration calls. This User
3rd Part Interface Name and Password is a “built-in” account that can be used in lieu of LDAP or local
user account authentication while submitting a URL call.

Configuring ViTALConnect 4.1.5 SSL

SSL Overview
Secure Sockets Layer (SSL) allows the server to provide secure data transmission over a network, necessary for
patient confidentiality and HIPAA compliance over the Internet.

Responsibility for SSL Management

The customer has responsibility for SSL management as part of their network security management. Should
they not have in-house expertise, the customer should identify and hire a security consultant. Vital Images has
a policy not to assume responsibility for the customer's network security.

CAUTION: If you do not use SSL in Internet Information Services (IIS) and ViTALConnect this system should
not be exposed to the Internet. It should only be run on the Local Area Network (LAN) or via a Virtual Private
Network (VPN) connection.

Pre-Installation Considerations:
Prior to going live with SSL, the customer must consider the following:

WAN Network Speed

Vital Images recommends a minimum connection speed of 2 Mbps (T1) from the client browser to the Internet.
The nominal and optimal server connection speeds are 6 Mbps and 45 Mbps, respectively.

External Access

The customer must modify their external firewall to open access to WAN clients on port 443 (for HTTPS/Secure
Sockets Layer). The firewall should be configured to route traffic on port 443 and port 8888 to the ViTALConnect
server.

Valid SSL Certificate

The customer must obtain a valid SSL certificate using one of two methods: (1) elect to have the SSL certificate
signed by a trusted third party Certificate Authority (CA) such as Verisign, Thawte, or Geotrust; or (2) choose to
sign their own SSL certificate with an internal CA.

NOTE: The customer site is responsible for tracking their own SSL certificates.

NOTE: The customer site should not store or keep their SSL certificates on the ViTALConnect. The files contents
are uploaded into the ViTALConnect database.

Power Supply for ViTALConnect server

Vital Images recommends that the ViTALConnect server be connected to an uninterruptible power supply (UPS)
or other constant power source. If power to the ViTALConnect server were to fail, all WAN Viewer services would
stop.

Portal Access

To ensure smooth SSL operation, customers are encouraged to direct users to a portal site that contains a link
to the ViTALConnect server. Users accessing the ViTALConnect server directly using HTTP will encounter different
behavior than users who access the server using HTTPS. Firewalls or routers can also be configured to redirect
HTTP requests (port 80) to the ViTALConnect server to HTTPS requests (port 443).

Secure Socket Layer Configuration and Policies

The Benefits of SSL

ViTALConnect’s HTTP transport, combined with SSL and Firewalls, provides the following benefits:

• Allows client to securely access ViTALConnect from the public network.


• Does not require the administration overhead of the VPN.
• Provides additional protection against worms that can invade from VPN-based solutions.

Obtain an SSL Certificate


A valid SSL certificate will need to be obtained. If this is a public facing device intended for use on the Internet
then a certificate from Thawte or Verisign will be the best choice; otherwise, the users will have to install the
certificate in the client browser manually.
1. Choose a Certificate Authority (CA).

Thawte and VeriSign are commonly used vendors of SSL certificates.


2. Generate a Server Key.

The server key is required to generate the Certificate Signing Request that is submitted to the Certificate
Authority. Each server key generated is unique. The key should be backed up to a secure location. If the key
is lost a new certificate will have to be obtained. This key must be kept private.
3. Generate a Certificate Signing Request (CSR).

Generate a Certificate Signing Request (CSR) using the server on which the SSL certificate will be installed.
The contents of the CSR file are needed when applying for the SSL certificate. It is critical that the
information in the CSR exactly matches the information in the Proof of Right. The most common Proof of
Right document is the Articles of Incorporation. As part of the CSR, the server's "Common Name" (CN) must
be entered. The CN is the fully qualified external hostname of the server. This is the hostname that remote
users will use to access the server. For instance, if remote users access the server as:

https://vitalconnect.myhospital.com, the Common Name will be

vitalconnect.myhospital.com

Do not include the https:// at the beginning of the Common Name.


4. Applying for the SSL Certificate.

Follow the instructions on the vendor's website to apply for the SSL certificate. Vital Images recommends
purchasing a 2-year certificate instead of a 1-year certificate, to avoid having to renew on a yearly basis.
Information on certificate renewal is provided by the Certificate Authority. See "SSL Certificate Expiration" in
the Troubleshooting section below for details on the impact certificate expiration has on the user
experience.
5. Installing the SSL Certificate.

Install the SSL certificate on the ViTALConnect server. Perform tests to verify that a user outside the
enterprise network can access the ViTALConnect client.

Troubleshooting

SSL Certification Expiration

If a certificate expires (and is not renewed), the user is presented with a warning dialog box indicating that the
certificate has expired. The user will be prompted with the following choices:

• Accept the certificate once


• Accept the certificate always
• Reject the certificate
An expired certificate has almost no impact on the security of SSL. Other than the warning dialog box, the
ViTALConnect application will continue to function normally. To avoid potential user confusion, renew your site's
SSL certificate prior to its expiration date. Details on the renewal process are provided by the Certificate
Authority that issued your certificate.

Hostname Mismatch
If there is a mismatch between the hostname ViTALConnect is connecting to and the hostname on the
certificate, a warning dialog box will appear, prompting the user to indicate whether to proceed or not.
For example, if a certificate has the hostname vitalconnect.myhospital.com and the configured host is
10.0.0.29, this error will occur, even if the "vitalconnect" host resolves to 10.0.0.29. When prompted to proceed
in such a case, the user must select [Yes]. If the user selects [No], the application will not launch.

Enable SSL on the ViTALConnect Server


To enable SSL, access the Administration pages:

Enable SSL on Internet Information Server


You must enable SSL on IIS on the Primary server to fully activate SSL on ViTALConnect.

NOTE: You cannot use both IIS and ViTALConnect encryption on a server running Windows XP. You can only use
ViTALConnect encryption.

To activate SSL on IIS:


1. From the Start menu, select Settings | Control Panel, double-click Administrative Tools, and double-click
Computer Management.
2. Select Services & Applications | Internet Info... | Web Sites | Default Web Site.
3. Right click on Default Web Site and select Properties.
The Default Web Site Properties dialog box displays.
4. From the Directory Security tab, click Server Certificate.
The SSL wizard displays.
5. Select either Create a New certificate (if you don’t have a certificate), or Assign Existing (if you have a
certificate)
• To create a new certificate, refer to Microsoft article #228821, “Generating a Certificate Request File
Using the Certificate Wizard in IIS 5.0,” then go to step 6 below.
• To create a new certificate, refer to “Securing Internet Information Services 6.0,” then go to step 6
below.
• To assign an existing certificate, go to step 6 below.
6. Navigate to the issued certificate.
7. Click Finish to complete the wizard.
• To access the secure server, always use “HTTPS” in the ViTALConnect access URL.
• For security reasons, port 80 should be blocked on the firewall to prevent access to the unsecured
server.

Export a Certificate/Key Pair from an IIS with SSL Certificate Installed

To Add Certificate Snap-in into Microsoft Management Console


1. From the Start menu, select Run and type mmc.
The Console 1 page displays.
2. Select File | Add/Remove Snap-in...
The Add/Remove Snap-in page displays.
3. Select Certificates and click Add.
The Certificates snap-in page displays.
4. Select Computer account and click Next.
The Select Computer page displays.
5. Select Local computer: (the computer this console is running on) and click Finish.
6. Click Close.

To Export the SSL certificate for key extraction


1. From the Console 1 page, select Certificates (Local Computer) and select the Personal folder.
2. In the Object Type section, double-click the Certificates folder.
The Web server certificate displays and is denoted by the “cn” (common name) found in the Subject field of
the certificate.
3. Right-click on the server certificate, select All Tasks, and click Export.
4. Export the server certificate to the desired directory (e.g., c:\).

NOTE: Use the .PFX format when exporting the server certificate and include the private key.

To Extract the Private Key File from the .PFX File

NOTE: Prior to running the commands listed below, make sure that \Program Files\HInnovation\Image Server is
defined in %PATH%.
• In the “Command Prompt” window, enter openssl pkcs12 -in <sourcefile> -clcerts -nokeys -out <destfile>.

EXAMPLE: Openssl pkcs12 -in c:\VitalImages.pfx -clcerts -nokeys - out VitalImagesPublic.pem

1. Click Browse next to the Certificate File (not specified): field.


2. Browse to and seelct the extracted public key (e.g. c:\VitalImagesPublic.pem).
3. Click Browse next to the Key File (not specified): field.
4. Browse to and select the extracted private key (e.g. c:\VitalImagesPrivate.pem).

5. Complete the SSL fields on this page. Refer to the steps below for detailed information on obtaining and
activating SSL encryption certificates and keys.

NOTE: A certificate file and key file are required to enter the below fields.
• Clear: Does not use HTTPS protocol and SSL.
Https port Port the ViTALConnect server uses to communicate via HTTPS. Verify that the default entry 443
is present. This is the default port that IIS uses; to change this port assignment, consult the IIS
documentation and enter the new port number.

Encrypt • Checked: Encrypts only the DICOM header (contains all patient information). This option
sensitive encrypts only the patient DICOM data, not the image, but offers the highest performance.
patient
info only • Clear: Encrypts both DICOM header and image data. This is the most secure option, but
uses the most server resources.

Certificate Navigate to the location you saved the certificate file and select the file (will have a default
File extension of *crt).

NOTE: You can generate your own certificate and key using an open source method (see
www.openssl.org), or purchase one from an authority such as Verisign (www.verisign.com).
Key File Navigate to the key file location.

6. Check the Use SSL and HTTPS box.


7. Enter or verify that 443 is entered in the Https port field.
8. Check or clear the Encrypt sensitive patient info only box (see the table above for details).
9. Browse to the location of the certificate file, and select the file.
10. Browse to the location of the key file, select the file, and click Save.
11. Stop the ViTALConnect server by selecting Start Menu | Programs | ViTALConnect | Stop Server.
12. Restart the ViTALConnect server by selecting Start Menu | Programs | ViTALConnect | Start Server.
13. Proceed to the next section, Enable SSL on Internet Information Server.

NOTE: You must also enable SSL on the Internet Information Server or SSL activation will be incomplete.

SMTP Server

Overview

Simple Mail Transfer Protocol (SMTP) is the standard for email transmissions across the internet.SMTP is a
relatively simple, text-based protocol, where one or more recipients of a message are specified (and in most

Licensing Manager

Configuring the ViTALConnect License

To configure the ViTALConnect license:

Vital Images will configure your ViTALConnect license and give you the license key.
1. On the left side of the Welcome to ViTALConnect Server Config page, click Licensing Manager.
The Licensing Manager page displays. Once you have configured the ViTALConnect license, the page
displays information about the number and types of users that your license allows access.

2. Obtain the MAC address (for each node in the load balancing configuration, if applicable): from the Start
menu, select Programs | Accessories | Command Prompt.
A command window opens.
3. In the command window, type ipconfig/all, then press Enter.
4. Find the physical address for the active ethernet adapter.
5. On the Licensing Manager page, enter the physical address into the Mac Address field, using all caps and
omitting the hyphens.

NOTE: For multiple servers, enter the MAC address of each node in the configuration, and separate each with a
dash.
6. Close the command window.
7. In the License Key field, enter the license key supplied by Vital Images.

8. Click to test that the license information is valid.

9. Click to save the license information.

Server Monitor

Overview
Use the server monitor configuration page to set how often the monitor checks the status of the server, and disk
space percentage and size. If the server is down, the monitor will reboot the server.

Configuring the Server Monitor

To configure the Server Monitor:

Client FTP

Overview

FTP or File Transfer Protocol is used to transfer data from one computer to another over the Internet, or through a
network. Specifically, FTP is a commonly used protocol for exchanging files over any network that supports the
TCP/IP protocol (such as the Internet or an intranet).

There are two computers involved in an FTP transfer: a server and a client. The FTP server, running FTP server
software, listens on the network for connection requests from other computers. The client computer, running
FTP client software, initiates a connection to the server. Once connected, the client can do a number of file
manipulation operations such as uploading files to the server, download files from the server, rename or delete
files on the server.

Configuring the Client FTP

To configure the Client FTP:


You use the client FTP configuration page to set up the server IP address and port.
1. On the left side of the Welcome to ViTALConnect Server Config page, click Client FTP.

Server Load Balance

Introduction

NOTE: You must have an installed load balancing system with multiple nodes to use this option. If you have a
single-server system and check this option, your ViTALConnect system will not function properly.

A load balanced system consists of one Primary server and one or more Rendering Nodes. You will need to
configure each node in the system slightly differently. The settings you enter here depend on the type of node
you are configuring (Primary or Rendering Node), the topography of the system, and the setup as specified on
page 11 of the ViTALConnect 4.1. Installation Guide.

Router Configuration

Before configuring the load balanced nodes, you must first configure your router and internal network as
specified on page 11 of the ViTALConnect 4.1. Installation Guide. The settings you enter here are determined by
the router configuration.

Configuring a Load Balancing Server

To configure load balancing


1. On the left side of the Welcome to ViTALConnect Server Config page, click Server Load Balance.

Consider main server for load Selected: The load balanced system uses the main (Primary) server to share processing
balancing load functions, 2D MPR as well as 3D.
Clear: The load balanced system does not share processing functions with the Primary
server. Rendering Nodes perform 3D processing; the main server performs database,
communication, and 2D processing.

Static router configuration Selected: Primary server uses a static IP address, which does not enable fault tolerance.
All Dynamic router configuration settings below this point are disabled.
Clear: Dynamic router configuration is selected (see below).

Main Server IP Static IP address of the main (Primary) server.

Dynamic router configuration Selected: Enables fault tolerance. Enter Dynamic router configuration settings below.

2. The AD Administrator must create a system account that has access for LDAP authentication. The account is
used by the ViTALConnect server to authenticate Active Directory users.
3. The AD Administrator adds the users to the proper group(s).

Alias The name of the remote DICOM server. This should be an easy-to-remember common
name.

AE Title The AE (Application Entity) name of the remote DICOM server.

Host Name The computer name/IP address where the remote DICOM server is installed.

Port Number The network port to which the remote DICOM server is listening.

Comments Enter comments, such as the name of the remote DICOM server's vendor.

3. Click Test to test the remote DICOM server.


If you receive the Test successful! message, the remote DICOM server is available. If you receive the Test
failed! message, either the information you entered for the remote DICOM server is wrong or the server is
not available.

4. Click to save the new DICOM server.


The new remote DICOM server information displays in the Remote AE Title list.
5. Click Main Page to return to the ViTAL ECF Management menu.

Installing PACS Server or Modality Using VICommunicator

VICommunicator Overview

VICommunicator is a Windows service that ViTALConnect software uses for DICOM communications.
VICommunicator is accessible through an interface that allows you to manage DICOM entries, customize the
autodelete function, and rebuild the patient database.

Testing the Functionality of VICommunicator

The VICommunicator service has a Graphical User Interface (GUI) that is installed with the software. The service
can be stopped and started through either the GUI or Windows Services.

To test the VICommunicator service:


1. Double-click on the small blue “V” icon on the toolbar (in the bottom right corner of the ) to launch the
VICommunicator window.

OR

Run ecfadmin from the Run window.

2. Click Test.

Restarting VICommunicator

Verify that the VICommunicator test response indicates “Running”. If the test bar indicates “Not running”,
restart the VICommunicator.

To restart the VICommunicator:


1. From the Start menu, select Control Panel I Administrative tools I Services.
2. Restart VIServices.
VIServices starts VICommunicator.
3. Make sure the blue “V” is on the task bar, and verify that VICommunicator is now running.

Volume Archive and DICOM Archive Locations

The Volume Archive is the location for the AVOL storage listed under PatientCabinet in the ECF configuration file.
The DICOM Archive is the storage location for the DICOM files, listed as the last PatientCabinet node in the
configuration.

This is specifically useful for ViTALConnect sites that have the Volume and DICOM archives listed in different
locations. There can be multiple volume archives, but only one DICOM archive.

The VICommunicator Config screen displays the location of the Volume Archives and DICOM Archive. Also, there
are fields to edit the Dropbox Location and Thumbnail Location.

Adding DICOM Devices

Overview

DICOM Printers, PACS, Vitrea, and other ViTALConnect workstations can be configured for communication.
VICommunicator uses wizards to set up these devices. It is also possible to make customizations and change
printer settings in VICommunicator.

Sending Images to the ViTALConnect

Vital Image’s DICOM Conformance statement outlines the image types the ViTALConnect workstation has the
ability to store. ViTALConnect Software will receive DICOM images without the sending device being added to
the software. There is no entry needed for any Scanners or PACS that are sending to ViTALConnect. By default,

ViTALConnect software is configured to receive only the following Storage Classes: CT, MR, Secondary Capture
(SC), and Presentation State (PR). Other Storage Devices need to be manually added.

Extended Storage Classes

To add extended Storage Classes:


1. Double-click the small blue “V” icon on the toolbar (in the bottom right corner of the ) to launch
VICommunicator window.

OR

Enter ecfadmin in the Run window.


2. Click DICOM.
3. On the Config tab, select the Local entry.
4. Click Edit to edit ViTALConnect’s local AETitle information.

5. Click Storage Options to select Storage Classes.

6. Select the Storage Classes boxes for local ViTALConnect. If no checkboxes are selected, ViTALConnect
will only receive images of Standard Storage Classes (CT, MR, SC, PR). Check all four Standard Storage
Classes before any Extended Storage Classes. Vital Images does not limit the number of Extended
Storage Classes selected.
7. Click OK.
8. Close the DICOM Server Edit window and VICommunicator.

Configuring ViTALConnect’s DICOM Settings

The default DICOM settings are VCECF for the AETitle and Port 3002. Both of these settings are customizable, but
Vital Images recommends you set the Port at 3002.

To change the local ViTALConnect AE Title:


1. Double-click the small blue “V” icon on the toolbar (in the bottom right corner of the ) to launch
VICommunicator window.

OR

Type ecfadmin in the Run window.

3. On the Config tab, select the Local entry.


4. Click Edit to edit ViTALConnect’s local AE Title information.
5. Edit the AE Title field.

NOTE: Vital Images recommends that you not change information into the Alias, Host, PDU, Port, SC
Alias fields.

NOTE: Vital Images recommends that you do not use the Auto Forward or Archive Server checkboxes.
6. Click Save.
7. Close the DICOM Server Edit window and VICommunicator.

Turning on the Multi-frame Option in ViTALConnect

You can configure ViTALConnect to accept multiframe CT or MR images.

To turn on the multi-frame option:


1. Double-click the small blue “V” icon on the toolbar (in the bottom right corner of the ) to launch
VICommunicator window.

OR

Type ecfadmin in the Run window.


2. Click DICOM.
3. On the Config tab, select the Local entry.
4. Click Edit to edit ViTALConnect’s local AETitle information.
5. Click Storage Options to select Storage Classes.
6. Check the Multi-frame Computed Tomography (CTMF) and Multi-frame Magnetic Resonance (MRMF)
boxes.
7. Click OK.
8. Close the DICOM Server Edit window and VICommunicator.

Configuring PACS in ViTALConnect

ViTALConnect sends to and queries/retrieves from PACS (Picture Archiving Communication System). Set up the
ViTALConnect configuration information as a DICOM entry on the PACS so that query/retrieve functions properly.

Be sure you have the following information before you begin:

• AE Title
• IP address
• Port number of the PACS

To add a PACS entry:


1. Double-click the small blue “V” icon on the toolbar (in the bottom right corner of the ) to launch
VICommunicator window.

OR

Type ecfadmin in the Run window.


2. Click DICOM.
3. On the Config tab, click New to add the New PACS entry.

6. On the Archive Configuration , click Yes to setup query and retrieve of the PACS.
7. Click Next.
8. Enter the information in the Alias, Host, AE Title, and Port fields.
The Alias field is how the entry displays in the software.
9. Click Finish to continue.
10. Close VICommunicator.
The setup of the PACS is complete.
11. Start ViTALConnect software to test.

Configuring Other VICommunicator DICOM Server Edit Options

Use the DICOM Server Edit , shown below, when you want to configure ViTALConnect to receive photos from a
Vitrea node.

After saving the new entry, the VICommunicator Admin screen displays.

9. Select the appropriate printer under the Printer: drop-down arrow.

NOTE: The default settings will work for most printers, you may need to make changes in some cases.
DICOM Printer’s configuration settings will be listed in the DICOM Conformance statement of the printer.

Compress Ratio
You can change the compress ratio for existing modalities, or add a new modality and compress ratio. The
higher the compression ratio, the more the image will be compressed; the ratio must be greater than 20.

Setting the Compress Ratio

To set the compress ratio:


1. On the ViTAL ECF Management menu, click Compress Ratio.
The Compress Ratio page displays.

2. Set the compression ratio for each image type, as well as the default ratio that is to be used for all other
images.
3. Click Save to save the values.

DICOM Query

Performing a Local DICOM Query

To perform a local DICOM query:


• Query the local server for DICOM worklist information and to send and delete studies, series, or images.
1. On the ViTAL ECF Management menu, click DICOM Query.
The DICOM Query page displays.

The default query will look for all DICOM information (studies). Blank fields represent a value of all.
Alternatively, you can enter query criteria in the following fields:
• PatientName
• PatientID
• AccessionNumber
• Modality
• Date of Last Study
2. To run the query, click .
The query can return three possible lists of DICOM data residing on the local server: studies, series, and
images.
• To sort the series list by a particular column, click the column header (such as “Patient Name”).
• To see the series in a study, click the study.
• To see the images within a series, click the series.
• To select or deselect all the items in the corresponding list, check or clear the Select All box.

Exporting a selected study, series, or image to a remote DICOM server

To export a selected study, series, or image to a remote DICOM server:


1. Select the item(s) you want to export by checking the box(es) in the left column of the corresponding list.
2. Select the server to which you want to send the item(s) from the dropdown list next to the Send To button.
3. Click Send To.

NOTE: The data is not sent instantly. The Store SCU service will process the export in the background.

Deleting a study, series, or image from the local server database

To delete a study, series, or image from the local server database:


1. Select the item(s) you want to delete by checking the box(es) in the left column of the corresponding list.
2. Click Delete by Schedule.

Scheduling a study, series, or image for automatic deletion the next time auto-delete runs

To schedule a study, series, or image for automatic deletion the next time auto-delete runs:

NOTE: You set the criteria for the auto-delete process on the DICOM Auto Deleting page.
1. Select the item(s) you want to schedule for deletion by checking the box(es) in the left column of the
corresponding list.
2. Click Delete by Schedule.
The Store SCU service will process the deletion(s) in the background.

To set up DICOM Forward Management:

Set up automatic forwarding to remote DICOM servers. Forward management will send all studies or just studies
with specific modalities that were processed on the client, then saved back on the server.

NOTE: This feature is ideal for PACS.

• Auto Forward indicates whether or not forwarding is turned on (Yes) or off (No) for the listed remote
server.
• Modality indicates the modality(ies) of images that are automatically forwarded to the listed remote
server.

Editing DICOM Forward Settings


1. To edit the settings for a remote DICOM server, select the radio button on the line associated with that server
and click .

2. Complete one of the following actions:


• To forward all images received by the local server, regardless of modality, leave the Modality dropdown
field set to All.

OR

• To forward only images the local server receives that belong to one specific modality, select a modality

• To turn forwarding on for the listed remote DICOM device, check the Auto Forward box.

• Configure the auto-delete criteria for the local server.
1. On the ViTAL ECF Management menu, click DICOM Auto Deleting.

• To configure the server to automatically delete files when disk space reaches a certain limit, complete the
following steps:
a Select Enable DICOM Auto Deleting.
b Accept the default size of 5000 MB, or enter a different size.

c Click .
When the disk space used reaches the specified size, the DICOM server will start deleting saved
images, starting with the oldest studies.

NOTE: Disk space is the amount of logical disk space remaining on the partition where the DICOM files
are saved, not the space left on the entire hard disk.
2. Click Main Page or click Here (if you clicked Save in 1c above) to return to the ViTAL ECF Management menu.

• Review the log of the DICOM devices sending the local server DICOM data.

To review reports:
1. On the ViTAL ECF Management menu, click Report Management.
The Report Management page displays.
2. Use the following pulldown menus to filter the results of your report query:

NOTE: Use the Patient Name field to enter patient information.

3. After selecting the desired criteria, click .

• Enter text into the Comment box and click Upload.
• Click Upload with an attachment and select a file to attach, then click OK.

server and control a variety of load balancing servers. This Chapter describes the following function, available
from the Load Balance Manangement menu:

• Click Control a Server or select the radio button and click Control a Server next to a desired server on the
Server Load Balance Status page.

• Click the Server IP: dropdown menu to select a different server to control.
• Current number of processing requests: lists the number of 3D requests being processed by the machine listed in the
Server machine’s IP address: dropdown menu.

NOTE: This field is updated every minute by default. It is dependent upon the number listed in the Registration
update time (seconds): field on the Server Load Balance within the ViTAL Connect Image Server Configuration
view.
• Server Enabled Status: lists whether the server machine listed in the Server IP: dropdown menu is enabled for 3D
processing. When it is enabled, the selected node will be considered for 3D processing sessions.

NOTE: If this option is enabled for the main server and if the Consider main server for load balancing load
checkbox on the Server Load Balance within the ViTAL Connect Image Server Configuration view is not checked,
no load will be sent to the main server.
• Reboot pending: lists whether a user has requested a reboot (via the Reboot Server or Reboot/Enable Server buttons)
and it is still pending for the machine listed in the Server IP: dropdown menu.

NOTE: This field is checked every minute by default. It is dependent upon the number listed in the Registration
update time (seconds): field on the Server Load Balance within the ViTAL Connect Image Server Configuration
view.
• Autoenable pending: lists whether a user has requested a reboot (via the Reboot/Enable Server button) and it is still
pending for the machine listed in the Server IP: dropdown menu. This field will switch the server to Enable when it
reboots.

NOTE: This field is checked every minute by default. It is dependent upon the number listed in the Registration
update time (seconds): field on the Server Load Balance within the ViTAL Connect Image Server Configuration
view.
• Restart pending: lists whether a user has requested a restart (via the Restart Image Service button) and it is still
pending for the machine listed in the Server IP: dropdown menu.

NOTE: This field is checked every minute by default. It is dependent upon the number listed in the Registration
update time (seconds): field on the Server Load Balance within the ViTAL Connect Image Server Configuration
view.

• Last time this server registered (or table was updated): lists the last time the server machine listed in the
Server IP: dropdown menu was registered. This value also is updated when the Disable, Restart Image
Service, Reboot Server, or Reboot/Enable Server button is clicked.

NOTE: If this field is not updated within the time listed on the It is dependent upon the number listed in
the Registration update time (seconds): field on the Server Load Balance within the ViTAL Connect
Image Server Configuration view, then either the server is not running or there may an issue.
• Last time an image was requested from this server: lists the last time a 3D image was requested for the
machine listed in the Server IP: dropdown menu.

NOTE: This field can be used to track whether this server is being used for processing 3D rendering
sessions by the load balancer.
• Server with GPU box indicates if there is a GPU on that server. Usually only the server with the GPU can be
used to render 3D.
• Affinity For MainServer: dropdown menu is not used at this time.
• Click Save to save the changes you have made on the .
• Click Refresh to refresh the values on this .
• Click Disable/Enable to toggle between enabled or disabled for consideration for 3D processing by the load
balancing algorithm.
• Click Restart Image Service to stop the Image Server Service for the machine listed in the Server IP:
dropdown menu.

NOTE: This field is checked every minute by default. It is dependent upon the number listed in the
Registration update time (seconds): field on the Server Load Balance within the ViTAL Connect Image
Server Configuration view.

Third Party Integration

Overview
Traditionally, separate systems working independent of each other provide key components within a diagnostic
imaging department. For example, Picture Archiving and Communications Systems (PACS) are used as the
primary means for diagnostic image review; while specialized post processing applications such as
ViTALConnect are often purchased to compliment the PACS workstation by providing high end 3D reconstruction
functions.
In an effort to redesign the traditional workflow to interpret diagnostic exams, Vital Images has introduced
Uniform Resource Locator (URL) integration capabilities for ViTALConnect 4.1.5. URL based integration assists in
the consolidation of the traditionally heterogeneous applications used throughout the diagnostic imaging
department within a single desktop interface; reducing the need for additional Personal Computer (PC)
hardware and improving workflow for the physician.
As most PACS, EMR, RIS, and other healthcare technology vendors have native capabilities to dynamically
generate an "in context" URL for integration purposes, the ViTALConnect third party integration capability
facilitates a simple mechanism for:

• Enabling third party applications with intuitive access to real-time images, a crucial part of the patient's
medical history. 3D, MPI, and MPR tools are launched directly from the EMR with a single button click or
menu selection.
• Snapshot restore capabilities.
• Direct access, with single sign on , to the ViTALConnect visualization tools, dramatically reducing reading
turnaround time while maintaining user productivity.
• Consolidation of clinical applications on a single workstation in the reading room.
• Automatic transfer of study identifiers to ViTALConnect to begin 3D post processing "in context." Eliminating
error-prone manual input of image identifiers in multiple applications.

Prerequisites
The ViTALConnect Third Party Integration interfaces all are capable of supporting a remote DICOM retrieve from
the list of configured DICOM Archive Servers, if the series requested is not already resident on the ViTALConnect
server.
To enable the remote DICOM retrieve aspect of this feature, you must install the VIMS Gateway service on the
ViTALConnect server.

NOTE: The VIMS Gateway must be installed on the server that the ECF component was installed on, which is
typically the main ViTALConnect server.
If you do not install this service, the interfaces will allow you to open series if they are already present on the
ViTALConnect server, but no remote DICOM retrieve functionality will work. Note that this means that the
FetchSeriesBy* interfaces will not operate.

To install the server, execute either the setup.exe or VIMSGatewayInstaller.msi from the ViTALConnect install
media in the VIMSGateway folder, and follow the instructions in the dialogs. You may run the installer at a later
time to repair or ununinstall the service.
The VIMS Gateway exposes a web service which provides a programmable means for optimally routing
information to the ViTALConnect server. Refer to the VIMSGatewayAPIAndInstallation.doc file in the same folder
for further details.

XML-Based URL Call


ViTALConnect 4.1.5 supports XML-Based Call to invoke any 3rd party interfaces. XML-Based URL Call can be
invoked in either secure or unsecure mode.

To invoke a ViTALConnect 3rd party interface in unsecure mode:


1. Build the request xml string
2. Invoke the URL call and post the request xml to the url. With the xml data param as "id"

NOTE: Refer to the “Sample URL Invocations, POST example code” section on page 69.

2. Encrypt the xml with Secure URL Builder
3. Invoke the URL call and transfer an encrypted request to url.

parameter parts (after =) with corresponding parameters. Those parameters are case sensitive. A sample

Since the parameters may contains special characters like &, < > / may break the XML formation, we recommend
avoid using such characters or use a xml parser to create the request xml string.

Encrypting the XML with Secure URL Builder


Secure URL Builder includes a Web Service and a Web proxy page to encrypt the request xml string for invoking
the 3rd party interfaces in a secure way.
Both the Web Service and the Web proxy page are only accessible via HTTPS.

Web Service: Secure URL Builder


The Web Service (https://<Server>/VCSURLBuilderWS/WebService.asmx ) only contains the following method:

Patient Reconciliation
CAUTION: Limited Patient Reconciliation is supported by substituting information passed in for the DICOM
information in the ViTALConnect client. Note that this does not modify the original DICOM data. Also, note
that a conference joiner will not see the reconciled information.
Patient Reconciliation should be only used with the OpenSeriesByStudyUID method. To use it, please follow the
steps:
1. Store the patient reconciliation information to a local file, e.g. C:\sample.xml.

Debugging URL Integration


You may temporarily enable web server supported debugging of the URL integration to diagnose issues. You
may enable the following logging facility to perform this function.

There are five interfaces provided by ViTALConnect control to open series directly.

1. OpenSeriesByAccessionNumber
Description
Remote query/retrieve is supported for this method. If the specified study/series is not found on local server,
image server will query from archive servers. If the study/series is found on one of the archive servers, image
server schedules a retrieve request from the first archive server on which the series is found.

NOTE: The archive server should be set correctly to enable auto-retrieve/fetch functionality. The archive must
support SCP CMove and SCP CFind service.

How to set archive server:


You can set one DICOM server as the archive server using one of the following methods:

• Within VICommunicator, navigate to the DICOM Server Edit screen and check the Archive Server box.

Login and Authentication


The ViTALConnect Third Party Integration interfaces allow a site to specify user account using a variety of
mechanisms. Uniquely identifying the originating user provides the following benefits:

• User Preferences: Customized user preferences will be maintained by user.


• Identification/Auditing: The specified user will be identified in features such as “email to clinician”, and in
the ViTALConnect study statistics and logging.
• Security: The user will be authenticated using appropriate credentials for your enterprise.
Note that the simplest integration uses a single, global user account for all requests. This may work for initial
test, or a small user base, but it is strongly recommended that either an LDAP or Alternate 3rdParty
authentication integration be implemented to achieve the full benefits.
The ViTALConnect Third Party Integration interfaces provide multiple mechanisms for authentication of the
request to suit different deployment and integration scenarios. Additionally, an encryption service is available
to protect credentials from being transmitted as a “clear text” URL (see “Encrypting the XML with Secure URL
Builder”) which is an important consideration when deploying an integrated solution for ViTALConnect for WAN
users (e.g. teleradiology).

No Authentication:

In the case that no extra authentication is required, omit the User and Pass XML attributes from the request. In
ViTALConnect server administration web pages, in the ViTALConnect Image Server Configuration page, you will
need to select a “proxy” account for these requests. (You may wish to create a new account specifically for this
purpose, e.g. “URLuser”.) Enter the account information in the Built-in Account for 3rd Party Interface: user
name and password, and press save.
All requests sent without User and Pass attributes will now use the account specified for authentication.

NOTE: If you are prompted to login, confirm that the “Force User Login” on the LDAP Integration is not checked.

Authenticate Using ViTALConnect User Database:

To authenticate a request against a the ViTALConnect User Database, insert the username and password into
the User and Pass XML attributes when you are forming the URL. If you intend to have all requests contain this
authentication information, you may clear the entries in the Built-in Account for 3rd Party Interface
administration page to disable the use of a “proxy” account when no User and Pass attributes are present (and
an request missing these fields will be fail authentication and be rejected.)

Authenticate Using LDAP or AD (ActiveDirectory):

If you are using an OpenLDAP or AD authentication service in your enterprise, and LDAP Integration has been
enabled on the ViTALConnect server (see “LDAP Integration”), you may specify this information by adding the
attribute Domain, e.g. Domain=“HEALTHNET”. You may optionally include the Pass attribute, if you wish to
provide a single-sign-on request. If the Pass attribute is omitted, you will be prompted to input your password. If
the specified domain is configured on the ViTALConnect LDAP Domain List, the credentials will be authenticated
using the specified server.

Alternate 3rd Party Authentication:

For some solutions, identity management may be provided by some other 3rd Party application or service (e.g.
an established PACS provider.) To support this scenario, the request can provide information to use an external
authentication provider by passing an AuthenticationURL attribute (containing the URL and associated

credentials) to a web site hosting the external authenticator. When ViTALConnect receives the request, it will
authenticate by invoking the AuthenticationURL, and checking the returned value.
To Authenticate against Third Party Authentication Server, authentication parameters should be specified in the
request xml - refer to the "Building the Request XML String" section on page 68 for details. All the parameters
listed are required.
When third party interfaces invoked, ViTALConnect Server will request the Authentication URL to validate the
session. The third party authenticate server should return a xml as the following:

RETURN_VALUE can be "Yes" for valid and "No" for invalid. The strings "Yes", "No" are case-insensitive when
ViTALConnect interpreting it.
If the session is valid, ViTALConnect will give the authenticated user (valid session) "Radiologist" privilege and
automatically generate a username based on the user information parameters (First Name, Last Name, Email
and so on). The user preference in ViTALConnect will be kept in ViTALConnect Server. When the user with same
user information invoke the third party interfaces again, saved user preference will be applied.

Image Compression Overview


As images are received by the ViTALConnect server they are automatically compressed using numerous layers.
These various levels allow ViTALConnect to display an image with progressively increased quality.
When configuring the system, a site will decide the quality level for initial display of the image as well as an
additional quality levels that will subsequently be displayed. For example, on a Wide Area Network (WAN) a site
might choose to display images with a diagnostic-quality compression without any further upgrades. On a Local
Area Network (LAN), the same site might configure the system to launch with diagnostic lossy data and
immediately upgrade to full-fidelity images (no user interaction required). These decisions are determined by
rules in the system known as interactive prefetch rules. Interactive prefetch rules are designed to allow sites to
configure viewing behavior around several environmental factors:

• LAN vs. WAN


• Modality
Based on these factors, the user can decide which compression levels are suitable for viewing.
For instance, sites that require only lossless viewing can alter their rules so that only the lossless image is ever
seen by the radiologist. It also allows sites to make their own determination as to what constitutes “diagnostic-
quality” lossless.

Overview
Each modality (e.g., CT, MR, etc.) has a specific Low and High compression ratio associated with it.
The higher the compression ratio, the more the image will be compressed. However, the more an image is
compressed, the more the visual quality worsens.

EXAMPLE: An image that is compressed at 30:1 will upload quicker across a LAN or WAN than an image that is
compressed at 10:1. However, the 10:1 image will have a much better visual quality.
As Administrator, you must define the common compression levels desired by the ViTALConnect users at your
location. If the users have a low bandwidth environment, they may want to receive low quality images at a fast
speed. If the users do not care about image quality, you can set the Low and High compression levels at 60:1
and 30:1 for example.
When you configure a ViTALConnect server, you define two compression levels: one for “review” and one for
“diagnostic compressed”. As the images arrive to the server, ViTALConnect creates three copies of the images:
one at review level, one at diagnostic compressed level, and one at full fidelity. Users can configure the server
to “stream” the images to them. For instance, a user can choose to receive the review version and then receive
the full fidelity images.
When an administrator changes these levels, it only impacts “new studies”. For example, if study A gets stored
as review compression of 30:1 and the user has configured his system to get “review” compressed images. A
year later the administrator changes review to mean 16:1. Study B arrives to the system and is compressed at
16:1 as per the new definition of review. Study A is still sent to the client at the old review meaning of 30:1.

Setting the compression ratios

The Compress Ratio allows you to modify the compression ratio for existing modalities. The default modalities
are CT, MR, NM, and PT.
You can also add a modality and define its default compression ratios or remove a specific modality from the
Compress Ratio .

Recommended Compression Level Defaults


Vital Images recommends the following compression level defaults for four common modalities:

4. Set the default ratio to be used for all other images.
5. Click Save to save the values.

Adding a Compress Ratio for an image type not listed

To add a compress ratio for an image type not listed:


1. From the Compress Ratio, click Add.

Overview
Prefetching is the ability to fetch images when a dataset is loaded in the background to optimize the user
experience. It is only beneficial for 2D reading mode. As Administrator, you enable prefetching on the system
level and then modify the prefetch capabilities for each user.
When prefetching is enabled, and a study is selected from the Study Directory, the images are cached into
memory. Depending on the compression ratio settings, images set to Low compression (default value of 10:1)
will load much faster than images set to High compression (default value of 30:1).
The section contains the following topics:

Enabling Prefetching
For images to take advantage of prefetching, you must enable the prefetch capability.

To enable prefetching

time a user wants to review these images again, they will be loaded from the local disc instead of server.

Settings for prefetch entire study


Overview
You can globally set that users are allowed to use study prefetching. You can also can set the upper
compression levels for various modalities to prefetch study in both LAN mode and WAN mode.
Listed below are three compression levels. Each level has a different compression ratio for each 2D image.

• Review - The lowest compression level for a lossy compression which has higher compression ratio.
• Diagnostic Compressed: The middle compression level for a lossy compression which has lower
compression ratio.
• Lossless Compression: The highest compression level for the lossless compression. This is the default
compression level for ViTALConnect image transfer.

Editing a Compression Level

NOTE: The default Modality is CT and its corresponding default Level is set to Lossless.
1. Navigate to the User Preferences Background Image Streaming.
2. In the Upper compression levels for prefetching a study, select a modality to edit and click Edit.

Settings for interactive prefetch


Overview
Interactive Prefetch is the ability to fetch images when a user interacts with images in the background to
optimize the user experience. Two levels of images (Low level and High level) should be fetched for each slice
during user interaction. The low level image should be fetched at first, and then the high level image. But if the
high level image is already fetched due to some reason, such as image is fetched by study prefetching, then the
low level image will not be fetched.

This procedure allows you to set the global setting for interactive prefetching. You can set the upper
compression levels (both low level and high level) for various modalities to do interactive prefetching in both
LAN mode and WAN mode.

NOTE: The default Modality is CT , with a default Low Res. level is set to Diagnostic , and its corresponding
default High Res. Level is set to Lossless.

Hardware Configuration Settings

Switching patient data from a non-shared drive to a shared drive


This section explains how to manage migrating ECF from using a non-shared drive to a shared drive. You would
complete this procedure when you use the hard drives on a server and set up a DAS or SAN to store ECF and
DICOM volume data.

To switch data from a non-shared drive installation to a shared drive installation


Complete this procedure to switch the data from a non-shared drive (such as C:\vitreadata) installation to a
shared drive installation for ECF (such as \\machinename\VitreaData).
1. On the new device, make a folder called VitreaData on the volume that you want to store all of the data.
2. Right-click the VitreaData folder and select Sharing.
3. Name the folder appropriately and set the security permissions.
4. Uninstall ECF on the Primary server.
5. Reboot the Primary server.
6. Reinstall ECF using the ViTALConnect CD. Use the share drive \\machine1\VitreaData (created in step 2
above) as the data drive (reboot if instructed by install program).
7. Double-click the blue “V” on the task bar. The VICommunicator screen displays.

Overview
CAUTION: This version of URL Integration is still supported in ViTALConnect 4.1.5. This document will be
deprecated in a future release of ViTALConnect.
Traditionally, separate systems working independent of each other provide key components within a diagnostic
imaging department. For example, Picture Archiving and Communications Systems (PACS) are used as the
primary means for diagnostic image review; while specialized post processing applications such as ViTAL
Connect are often purchased to compliment the PACS workstation by providing high end 3D reconstruction
functions.
In an effort to redesign the traditional workflow to interpret diagnostic exams, Vital Images has introduced
Uniform Resource Locator (URL) integration capabilities for ViTALConnect 4.0.1.0. URL based integration assists
in the consolidation of the traditionally heterogeneous applications used throughout the diagnostic imaging
department within a single desktop interface; reducing the need for additional Personal Computer (PC)
hardware and improving workflow for the physician.
As most PACS, EMR, RIS, and other healthcare technology vendors have native capabilities to dynamically
generate an "in context" URL for integration purposes, the ViTALConnect URL integration capability facilitates a
simple mechanism for:

• Enabling third party applications with intuitive access to real-time images, a crucial part of the patient's
medical history. 3D, MPI, and MPR tools are launched directly from the EMR with a single button click or
menu selection.
• Snapshot restore capabilities.
• Direct access, with single sign on , to the ViTALConnect visualization tools, dramatically reducing reading
turnaround time while maintaining user productivity.
• Consolidation of clinical applications on a single workstation in the reading room.
• Automatic transfer of study identifiers to ViTALConnect to begin 3D post processing "in context."
Eliminating error-prone manual input of image identifiers in multiple applications.

Description
Remote query/retrieve is supported for this method. If the specified study/series is not found on local server,
image server will query from archive servers. If the study/series is found on one of the archive servers, image
server schedules a retrieve request from the first archive server on which the series is found.

Notes for Using 3rd Party Interfaces


1. To use the URL based call, replace the parameter parts (after =) with corresponding parameters. Those
parameters are case sensitive.
2. In addition, you can specify user information in the URL, including Domain, UserName and Password. These
informations should be Base64 encoded.

And you can choose the following formation:

• All Domain, UserName, Password specified


Means 3rd part interface wants to login in as specified LDAP user and authenticate it with the password
provided.
• Domain and UserName
Means 3rd part interface wants to login in as specified LDAP user but do not want authenticate it.
• UserName, Password
Means 3rd part interface wants to login in as specified local user and authenticate it with the password
provided.
• None of the three.
For this case, if the "Built-in Account for 3rd Part Interface account" is set and "Force User Login" is
disenabled, ViTALConnect will use the built-in account to login. Otherwise, user should input login
information and click "Sign in" button to logon in the login web page.

Anatomy Protocols

Overview
When you load a volume or study, ViTALConnect selects an anatomy protocol based on the study description.
The protocol changes the color, opacity, and window/level to highlight relevant data elements for that exam
type. The default protocols are starting points; you can change the protocol to suit the exam type by selecting a
new one from the 3D Options pulldown menu.

ViTALConnect protocols and associated presets were developed by a leading institution in clinical radiology and
represent some of the foremost expertise in computer viewing of scanned images in the world today. However,
it is important that you regard each protocol and its views as a convenient starting point. As such, each protocol
may not necessarily provide the optimal viewing settings for any particular volume. For this reason,
ViTALConnect provides controls for fine-tuning each image.

Picking an Anatomy Protocol


You can keep the system-assigned anatomy protocol, or select a different one.

To select a protocol:

1. Click (if not already in 3D mode).


The Viewer changes to a 4-up, 3D mode.
2. From the 3D Options dropdown menu on the Visual tab, select the desired Anatomy Protocol from the
dropdown menu.
The presets associated with the selected protocol display as thumbnail preview images, as applied to the
