Worldwide, including JA Asia Pacific Limited and Junior Achievement Hong Kong, the right to

I grant JA Worldwide the irrevocable right and permission to:

1.) Reproduce, use, re-use, publish, display, copyright, and distribute all media taken in which I
might appear and in connection with any and all media now known or hereinafter developed.

2.) Reproduce, use, re-use, publish, display, copyright, and distribute the same, individually or in
conjunction with other photographs, images, text, and/or captions in connection with any
promotion, advertising, and publicity of JA Worldwide in all media.

I understand that the copyright to all media resides with JA Worldwide.

 Consent for use of Image (Parental Consent)

The above identified Undersigned acknowledges that they are the parent or guardian of the Child and
hereby grants, in such capacity on behalf of such child, consent to Federal Express (Hong Kong
Limited (“FedEx Express”) and its parent and affiliate companies (collectively “FedEx”, which includes
representatives, assigns, officers, directors, employees, or any other person or persons, corporation,
or corporations under whose authority FedEx or FedEx Express may be acting) the right to reproduce
and use the Child’s name, picture, image, likeness, comments, and information (collectively, “Image”)
provided while recording the Child. Such consent shall extend to any means available now or in the
future, in connection with FedEx Express marketing, promotional, communication and advertising
activities in any and all media channels, including but not limited to FedEx Express’ social media
channels, which may include but is not limited to, Facebook, YouTube, LinkedIn, Instagram, Twitter
(“Purpose”).

The Undersigned hereby relinquishes to FedEx Express all right, title, and interest, including
copyright, in the Image, and grant to FedEx Express the right to give, transfer, display or exhibit,
reproduce, and copy to any third party for the Purpose. The Undersigned acknowledges that FedEx
Express may make any and all changes and modifications, rearrangements, additions and deletions
to the Image, without their approval.

The Undersigned releases and discharges FedEx, from any liability that may arise from the taking,
process, reproduction or the use of the Image for the Purpose, including but not limited to any claim of
defamation.

If any provision of this release is held to be invalid, illegal, or unenforceable, the validity, legality and
enforceability of the remaining provisions will in no way be affected or impaired. This release shall be
governed by the laws of the jurisdiction, where FedEx Express has signed this release. This release
shall be construed and interpreted in accordance with the laws of said jurisdiction with no regard for
the principles on conflicts of laws. Any dispute relating to this release shall be submitted to the
exclusive jurisdiction of the courts of the jurisdiction where FedEx Express has signed this release.
The Undersigned represents and certifies that he or she has read the foregoing and fully understands
the meaning and effect thereof, is the parent or guardian of the Child identified above, and is legally
permitted and authorized to enter into this Consent on behalf of such Child, and intends to be legally
bound, executes this Consent for the Purpose and at the date mentioned herein.
