Dear Sir/Madam:

The Department of Public Works and Highways (DPWH), Regional Office VIII, Baras, Palo, Leyte, through the Bids and Awards Committee
(BAC) and thru GAA FY2016 DPWH INFRASTRUCTURE PROGRAM, intends to apply the sum stated below being the Approved Budget for
the Contract (ABC) to payments for the said contract, hereby notifies the above invited DPWH qualified Consultant, to bid/negotiate for the
herein contract through Alternative Method of Procurement under Negotiated Procurement (Two Failed Biddings), viz:

a. Contract ID : 16CSIO03 (Negotiated Procurement – Two-Failed Biddings)

b. Name of Contract : Subsurface Exploration / Geotechnical Investigation, Proposed Construction of Tide
Embankment, Section 5, Municipalities of Palo and Tanauan, Leyte
c. Project Description/Scope of : The Consultant shall: 1) Carry-out liquefaction analysis per borehole; 2) Conduct SPT at 1.0 meter
Work intervals; 3) One (1) sample every 1.0m depth; 4) Assumed 3 samples from each boreholes; 5) Max. 3-
meters coring depth each boreholes; & 6) Final geotechnical report shall be prepared in three (3) hard
copies and one (1) soft copy in DVD.
Total Number of Boreholes = 31 Boreholes; Boreholes Depth = maximum 30 m. depth
d. Min. No. of Drilling Equipment : Four (4) units
e. Estimated Project Cost/ABC : ₱ 2,614,659.35
f. Duration : 80 Calendar Days (including mobilization/demobilization/laboratory work)
g. Cost of Bidding Documents : ₱ 5,000.00
Please be informed that your firm is among the three (3) invited DPWH Accredited Consultants who was initially found qualified/eligible by our
Bids and Awards Committee (BAC) as to legal, technical and experience requirements. The following are the invited Consultants:
1. E.B. TESTING CENTER, INC.
2. TERMS CONC. AND MATERIALS TESTING LAB., INC.
3. N-PRECISION CONST. MATERIALS TESTING CENTER
Bidding will be conducted through Rule XVI -Alternative Method of Procurement under Section 53.1 – Negotiated Procurement (Two Failed
Biddings), where there has been failure of public bidding for the second time, pursuant to the Revised Implementing Rules and Regulations
(IRR) of Republic Act No. 9184.
In view hereof, the procuring entity through the BAC request your Company to submit curriculum vitae and to bid/negotiate your best proposal
for the abovementioned project for our evaluation and post-qualification.
The procurement calendar of activities are listed hereunder:

3. Deadline for Receipt of Bids/Proposals February 15, 2017 until 1:30 P.M.
4. Opening of Bids/Proposal February 15, 2017 @ 2:00 P.M.
The BAC shall conduct Evaluation of Consultants’ Bids/Proposals using the Quality-Cost Based Evaluation (QCBE): Passing Technical
Scores and Quality-Cost Weights procedure. Only bids whose technical proposals pass the minimum technical evaluation score of seventy
five percent (75%) shall have their financial proposals opened and evaluated. The technical proposal shall carry eighty percent (80%) weight in
the evaluation. The criteria and rating system for the evaluation of bids shall be provided in the Instruction to Bidders (ITB).

A complete set of Bidding Documents may be purchased from the BAC Office upon payment of a non-refundable fee for the Bidding
Documents in the amount of ₱ 5,000.00. You may also download the Bidding Documents from the websites of the DPWH at
www.dpwh.gov.ph and the PhilGEPS at www.philgeps.gov.ph. Bidders/Consultants shall submit bids/proposals though their duly
Authorized Liaison Officers only.

Bids must be delivered at BAC Office, DPWH Regional Office VIII and will be opened in the presence of the bidders’ representatives who
choose to attend. Bids received in excess of the ABC shall be declared non-complying at bid opening. Late bids shall not be accepted.
The DPWH Regional Office No. VIII reserves the right to accept or reject any bid, to annul the bidding process and to reject all bids at any time
prior to contract award without thereby incurring any liability or obligation to the affected bidder/s.
