JOINT AFFIDAVIT OF CONSENT

(to adoption)

WE, LUIS BAURA y APOLINAR and MARIA ELENA BAURA y

PEREZ, married to each other, residents of 2451 Reverence St., Peace
Valley, Capitol Site, Cebu City, both Filipinos, of legal age, after having
been duly sworn to in accordance with law, hereby depose and state
that:

1. We desire EVANGELINE CRYSTAL GARCIA PEREZ to be our

own child and to carry our name;

2. EVANGELINE CRYSTAL GARCIA PEREZ has been under the

care and custody of her sister MARIA ELENA PEREZ BAURA from 14
June 2009 up to the present since the untimely demise of their parents,
SPS. PEREZ on the said date. When we got married, we brought her into
our conjugal home and jointly took care and custody of her;

3. From the time the minor has been living with us, we,
especially I, MARIA ELENA PEREZ BAURA, accepted and treated
EVANGELINE CRYSTAL GARCIA PEREZ not only as myher sister but as if
she was myher own child: I spent for the latter’s food, shelter, and other
necessary expenses;

4. We strongly bond with EVANGELINE CRYSTAL PEREZ

physically, emotionally, socially and spiritually. As a matter of fact,
EVANGELINE CRYSTAL GARCIA PEREZ treats us LUIS APOLINAR
BAURA and MARIA ELENA PEREZ BAURA as if we were are her parents;

5. I, MARIA ELENA PEREZ BAURA, love EVANGELINE

CRYSTAL GARCIA PEREZ, my sister, very much and I treat her as if she
were my own;

6. I, MARIA ELENA PEREZ BAURA, together with LUIS

APOLINAR BAURA would desire to adopt EVANGELINE CRYSTAL
GARCIA PEREZ as ourmy own child and to carry our name;

7. We have talked to EVANGELINE CRYSTAL GARCIA PEREZ

several times about our desire to adopt her, and she has always been
very excited about it and has also expressed to us her desire to be
adopted by us;

8. We have the financial means to provide the needs of

EVANGELINE CRYSTAL GARCIA REYES, as I, LUIS APOLIANR BAURA,
am gainfully employed and currently occupying the position of

President and Chief Executive Officer of DELEX PHARMA PHILIPPINES
with address at F. Cabahug St., Barangay Mabolo, Cebu City, and has
substantial investments in some corporations based in Cebu City; while
the I, MARIA ELENA PEREZ BAURA, am the owner of Eve Cuisine in
Cebu City;

9. By virtue of these presents, we hereby expressly give and

agree, and hereby expressly gave our consent to said adoption;

10. Our primary consideration in giving our consent to the

adoption is anchored on the paramount welfare and the well-being of
EVANGELINE CRYSTAL GARCIA PEREZ, as she deserves to have a
complete family with a father in her life; and

11. We are executing this Affidavit to attest to the truthfulness

and veracity of the foregoing statements; and in support of the Petition
for Adoption of EVANGELINE CRYSTAL GARCIA PEREZ; and for
whatever legal purpose it may serve.
