Practicum Orientation Guide

Congratulations you are now to begin your professional journey as an intern. You are about to experience, observe
and learn from actual work settings. Apply the good things that you have learned from your classes. Be guided also
by the following:
1. You are to complete 420 hours for the semester.
2. Before the start of your internship, submit the following:
a. Photocopy of the Endorsement Letter with acknowledgement of receipt from the cooperating agency.
b. Personal Data Sheet (PDS)
c. Parents’ Consent (It can be submitted to your instructor when classes start)
d. Long brown envelope for your file
3. Every grading you are required to complete 140 hours and submit the following:
Prelims:
a. Handwritten reports on your accomplishments, insights, challenges, observations, and values learned.
b. Photocopy of your time card with computation of the number of hours completing 140 hours.
c. Evaluation of your direct supervisor (See evaluation form).
Midterms:
a. Handwritten reports on your accomplishments, insights, challenges, observations, and values learned.
b. Photocopy of your time card with computation of the number of hours completing 140 hours.
c. Evaluation of your direct supervisor (See evaluation form).
Finals:
a. Encoded reports on your accomplishments, insights, challenges, observations, and values learned.
b. Photocopy of your time card with computation of the number of hours completing 140 hours.
c. Evaluation of your direct supervisor (See evaluation form).
d. Certificate of Completion to be issued by the cooperating agency specifying that you have completed 420
hours.
4. You are solely responsible for your actions. “Do not do things that may harm your reputation”.Be guided also
by the following rules:
a. Do not post pictures and any negative comments in your Facebook, instagram, twitter and social media
accounts. Once you do this, make a Letter of Apology to the institution and make a Letter of Explanation
to your practicum adviser and specify that you are willing to receive deductions or a grade of 65 for the
grading period.
b. Unexcused tardiness of 15 minutes and absences are penalized twice (2x). In case you are sick or you
attended an emergency, make an excuse letter to be addressed to your supervisor and to be noted by your
practicum adviser. For those enrolled in research, please ask your research adviser on the schedule of your
consultation hour and notify your supervisor and specify it in your PDS.
c. Be responsible to the tasks assigned to you. Ask your supervisor for concerns and matters related to your
task.
d. Stretch, take initiative and ask your supervisor for tasks that you can do when you are free. Accept tasks
such as filing, encoding, typing and other office works.
e. Smile and greet everyone in the office.
f. Always capture the lessons. You are responsible for your learning.
5. You have dignity as a person. Do not hesitate to report incidents to your practicum adviser or to your
department head things or incidents that are detrimental to you.
6. Grading system and other requirements will be discussed by your practicum adviser.
7. You are required to attend two (2) seminars to be organized by Jr. Finex. Schedule will be determined and
announced later. Save for an expected seminar fee and lunch of Php. 250.00.
8. You are required to attend recollection mass for graduating students. Schedule will be announced.
9. Fill-up the practicum directory and join the Facebook group “FinMan Practicum” for easier dissemination of
information.
